摘要

目的

观察耳穴压豆联合韦贵康经筋骨三维立体手法治疗膝骨关节炎（KOA）的临床疗效。

方法

选取广西中医药大学第一附属医院仁爱分院2023年1—12月收治的KOA患者60例，采用随机数字表法分为观察组和对照组各30例。对照组给予韦贵康经筋骨三维立体手法治疗，观察组给予韦贵康经筋骨三维立体手法联合耳穴压豆治疗，连续治疗4周。治疗前后用奎森功能演算指数（Lequesne）评分评估关节功能障碍程度，骨关节炎指数评估量表（WOMAC）评分评估膝关节功能，视觉模拟量表（VAS）评分评估疼痛程度，Lysholm评分评估膝关节功能状态与稳定性，站立-行走计时测试（TUGT）评估站立行走能力，静态平衡量表（BBS）评分评估平衡功能，5次坐站试验（FTSST）评估下肢肌力和动态平衡能力，对比2组治疗期间的安全性、治疗后的临床疗效。

结果

治疗后，2组VAS、WOMAC、Lequesne评分均较治疗前降低（

0.05），Lysholm评分均较治疗前升高（

0.05）；且观察组VAS、WOMAC、Lequesne评分低于对照组（

0.05），Lysholm评分高于对照组（

0.05）。治疗后，2组FTSST、TUGT均较治疗前降低（

0.05），BBS评分均较治疗前升高（

0.05）；且观察组FTSST、TUGT低于对照组（

0.05），BBS评分高于对照组（

0.05）。治疗结束后4、8、12周，观察组总有效率高于对照组（

0.05）。所有患者治疗期间均未发生明显不良反应。

结论

耳穴压豆联合韦贵康经筋骨三维立体手法治疗KOA疗效持久。

Abstract

Objective

To observe the clinical efficacy of ear acupoint pressing with beans combined with WEI Guikang's three-dimensional manipulation of the meridians， tendons， and bones in the treatment of knee osteoarthritis （KOA）.

Methods

Sixty KOA patients admitted to the Ren'ai Branch of the First Affiliated Hospital of Guangxi University of Chinese Medicine from January to December 2023 were enrolled and randomly assigned into an observation group and a control group （30 patients each） according to a random number table. The control group received WEI Guikang's three-dimensional manipulation treatment alone， while the observation group received the same manipulation combined with ear acupoint pressing with beans for 4 consecutive weeks. Before and after treatment， joint dysfunction was assessed using the Lequesne functional index. Knee function was evaluated with the Western Ontario and McMaster Universities Osteoarthritis Index （WOMAC）. Pain was assessed using the Visual Analog Scale （VAS）. Knee function and stability were evaluated with the Lysholm score. Standing and walking ability was assessed using the Timed Up and Go Test （TUGT）. Balance was measured with the Berg Balance Scale （BBS）. Lower-limb muscle strength and dynamic balance were evaluated using the Five Times Sit-to-Stand Test （FTSST）. Safety and clinical efficacy during treatment were compared between the two groups.

Results

After treatment， VAS， WOMAC， and Lequesne scores decreased and Lysholm scores increased in both groups compared with baseline （both

0.05）. The observation group showed greater improvement than the control group （

0.05）. FTSST and TUGT scores decreased， and BBS scores i

ncreased in both groups after treatment （both

0.05）. The observation group improved more than the control group （

0.05）. At 4， 8， and 12 weeks post-treatment， the observation group had a higher total effective rate than the control group （

0.05）. No significant adverse reactions occurred in either group during treatment.

Conclusion

Ear acupoint pressing with beans combined with WEI Guikang's three-dimensional manipulation of the meridians， tendons， and bones provides a lasting therapeutic effect in patients with KOA.
