摘要

扩张型心肌病是进行性的左心室或全心室的扩张或肥大，并伴有收缩功能障碍的心肌疾病，起病隐匿，预后不佳。中医将扩张型心肌病归属于“心胀”“心水”“喘憋”范畴。从“诸气膹郁，皆属于肺”阐释DCM的病机，急性期乃相傅之官失职，包括肺卫不固，外感邪气，内传心系；宣降失调，水饮内停，痰瘀胶结。缓解期勿忘心肺之本，包括肺气不足，营卫亏虚，损其心也；肺朝百脉，心肺之疾相互影响。急性期以宣、泻、降为上，包括宣肺解表、泻肺利水、降肺通腑。缓解期以益、润为先，包括益肺固卫、润肺养心、临证加减。

Abstract

Dilated cardiomyopathy（DCM） is a myocardial disease characterized by progressive dilation or hypertrophy of the left ventricle or the entire ventricle，accompanied by contractile dysfunction.It has a subclinical onset and a poor prognosis.In traditional Chinese medicine（TCM），DCM is classified under "heart distension"，"heart edema"，and "shortness of breath".According to the principle of "all symptoms of

stagnation and depression belonging to the lung"，the pathogenesis of DCM is interpreted as follows：in the acute phase，the interrelated organs fail in their functions，including deficiency of lung defensive

，invasion of pathogenic factors，and transmission to the heart system；dysregulation of ascending and descending，retention of water，and phlegm-blood stasis congealing occur.In the remission phase，attention should be paid to the root of the heart and lung，including lung

deficiency，insufficiency of nutrient and defense，and resulting heart impairment；since the lung governs all the vessels，heart and lung diseases mutually influence each other.Treatment in the acute phase focuses on promoting，purging，and descending，including dispersing the lung and releasing the exterior，purging the lung and promoting water metabolism，and descending the lung to open the bowels.In the remission phase，the emphasis is on tonifying and moistening，including benefiting the lung and consolidating defensive
