摘要

艰难梭菌相关性腹泻（CDAD）临床症状主要为发热、腹痛、水样便腹泻，严重者可引发假膜性肠炎，且常伴有中毒性巨结肠、肠穿孔、感染性休克等并发症，甚至导致死亡。本案患者经多次常规治疗后仍反复出现腹泻症状，属复发性CDAD。通过辨证论治，予其温补脾肾法治疗。治疗后患者腹泻频次明显减少，粪便性状逐渐恢复正常，腹胀、肠鸣等伴随症状显著缓解。中医药治疗复发性CDAD具有良好的临床疗效，为临床治疗此类疾病提供了新的思路与选择。

Abstract

Clostridium difficile associated diarrhea （CDAD） is clinically characterized by fever， abdominal pain， and watery diarrhea. Severe cases can lead to pseudomembranous colitis and are often accompanied by complications such as toxic megacolon， intestinal perforation， and septic shock， and may even result in death. In this case， the patient continued to experience recurrent diarrhea despite multiple conventional treatments， constituting recurrent CDAD. Through syndrome differentiation， the patient was treated using the method of warming and tonifying the spleen and kidney. After treatment， the frequency of diarrhea was significantly reduced， stool consistency gradually returned to normal， and accompanying symptoms such as abdominal distension and intestinal rumbling were markedly relieved. Traditional Chinese medicine treatment for recurrent CDAD demonstrated good clinical efficacy， providing new strategies and options for the clinical management of this condition.
