摘要

目的比较不同医疗保障制度下脑卒中患者的住院费用及报销比例,并分析影响医疗费用的因素。方法采用回顾性调查法,收集2015年北京中医类医院的住院患者第一诊断为脑卒中的病案,采用SAS 9.3软件描述性分析、单因素方差分析、χ2检验及多元线性回归方法分析患者的基本情况和费用。结果不同付费方式下总费用（F=13.37,P<0.01）、诊断费（F=25.05,P<0.01）、治疗费（F=12.14,P<0.01）、药品费（F=12.22,P<0.01）和住院费用构成（χ2=4 976.75,P<0.01）比较,差异均有统计学意义。城镇居民基本医疗保险和新型农村合作医疗自付费用较高,城镇职工基本医疗保险的住院费用高于城镇居民基本医疗保险、新型农村合作医疗结算的住院费用。结论医疗卫生管理部门应从药费、住院天数等方面重点监控住院费用,进一步推广社会保险,加大对公费医疗的改革制度,减轻脑卒中患者的经济负担。
