摘要

目的观察益气养阴、活血化瘀法治疗老年2型糖尿病肾病（Ⅲ、Ⅳ期）的临床疗效。方法将70例糖尿病肾病（DN）患者按年龄分成2组,比例约为1∶1,A组年龄<60岁,B组年龄≥60岁。均给予益气养阴、活血化瘀法为主治疗12周,观察2组疗效及治疗前后中医证候积分、空腹血糖（FBG）、餐后2小时血糖（2 hPG）、糖化血红蛋白、24h尿蛋白定量、肝肾功能指标、血脂指标等变化情况。结果服药12周后,A组总有效率82.35%,B组为77.78%,但2组间差异无统计学意义（P>0.05）。治疗后,A组中医症候积分及尿白蛋白排泄率、HDL-C、TG均较治疗前改善（P<0.05）;B组中医症候积分及尿白蛋白排泄率均较治疗前改善（P<0.05）。治疗后2组间比较,尿微量白蛋白排泄率差异有统计学意义（P<0.05）。结论对Ⅲ、Ⅳ期的老年2型DN患者使用益气养阴、活血化瘀法能够有效的改善临床症状,但控制蛋白尿改善肾小球滤过率方面效果不及非老年患者。
