摘要

幼年特发性关节炎（juvenile idiopathic arthritis,JIA）是儿童最常见的结缔组织病之一,属中医学"痹病""小儿痹"范畴。由于儿童处于快速生长发育阶段,不同年龄段其身体功能亦有不同,故JIA多表现为不同的临床亚型。目前认为,先天禀赋不足、脾肾亏虚是JIA发病的内因,湿、瘀、毒三者相互作用则是其进一步发展变化的关键。湿、瘀、毒既可由内伤产生,也可由外感导致,同时湿浊和瘀血既是致病因素也是病理产物,而从湿浊到瘀血、从湿瘀成毒到湿瘀毒互结是JIA病情进展的两个质变。因此,正确认识湿、瘀、毒三者的来源及其在JIA发病中的相互关系十分重要,有助于提高JIA辨证论治的准确性,治疗上也应当重视祛湿、化瘀、解毒法的运用,同时还需考虑不同年龄段患儿的生理特性。
