摘要

目的观察自拟健脾祛浊通络汤治疗慢性肛周湿疹脾虚痰瘀阻滞证的临床疗效,并探讨其可能的作用机制。方法选择2017年1月—2018年11月就诊的慢性肛周湿疹患者80例,按随机数字表法分为对照组与治疗组各40例。2组均采用复方亚甲蓝注射液肛周局部点状封闭治疗,对照组给予口服盐酸左西替利嗪片5 mg/d,肛周局部外用丁酸氢化可的松乳膏;治疗组给予自拟健脾祛浊通络汤内服与肛周熏洗治疗。2组均治疗4周。对比2组治疗前后瘙痒程度、皮损形态、皮损面积评分及皮肤病生活质量指数（DLQI）评分;检测2组治疗前后血清白细胞介素-4（IL-4）、白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）及总IgE;对比2组疗效。结果 2组治疗后瘙痒程度、皮损形态、皮损面积及DLQI评分均较治疗前降低（P<0.05）,且治疗组低于对照组（P<0.05）。2组治疗后血清IL-4、IL-6、TNF-α及总IgE水平均较治疗前降低（P<0.05）,且治疗组低于对照组（P<0.05）。治疗组总有效率高于对照组（P<0.05）。结论自拟健脾祛浊通络汤能够减轻慢性肛周湿疹患者的瘙痒程度,改善皮损状态,减小皮损面积,提高生活质量及临床疗效,其作用机制可能与降低机体炎症因子水平、抑制变态反应有关。
