摘要

目的观察丹参川芎嗪注射液对Ⅲ期非小细胞肺癌患者放化疗所致心肌损害的保护作用以及对血流动力学参数的影响。方法筛选Ⅲ期非小细胞肺癌患者80例,按照随机数字表法分为对照组和观察组,各40例。对照组参照规范给予三维适形放疗+TP化疗方案;观察组在对照组基础上给予丹参川芎嗪注射液,每次5～10 mL,静脉滴注,连续治疗6周。比较2组近期疗效、心肌损伤标志物水平、心功能、血流动力学以及不良反应发生情况。结果观察组的近期总有效率为37.50%,对照组为35.00%,2组差异无统计学意义（P>0.05）。治疗后,观察组患者治疗后心肌损伤指标血清肌酸激酶（CK）、肌酸激酶同工酶（CK-MB）、肌钙蛋白I（cTnI）、氨基末端脑钠肽原（NT-proBNP）水平明显低于对照组;心功能指标左室心肌质量指数（LVMI）、平均室壁应力（MWS）亦低于对照组,差异均有统计学意义（P<0.01）。治疗后,观察组全心舒张末期容积指数（GEDI）、全身血管阻力指数（SVRI）、血管外肺水指数（EvLwI）水平明显低于对照组,心脏排血指数（CI）明显高于对照组,差异均有统计学意义（P<0.01）。观察组的血栓发生率为7.5%明显低于对照组的30.0%,差异有统计学意义（P<0.05）。结论丹参川芎嗪注射液对Ⅲ期非小细胞肺癌放化疗患者的心肌损伤有保护作用,且可有效改善患者的血流动力学参数。
