摘要

功能性便秘以肠道动力异常为主要表现,并常伴有精神心理异常,属脑肠互动异常,以脑肠轴为基础,表现为胃肠动力异常及伴随的精神心理异常。肝主疏泄,主要表现在调畅气机和调节情志方面,在便秘的发生发展中具有重要作用。以肝失疏泄为病理基础辨证分型,从肝气郁滞、肝气逆乱及兼顾调畅情志等方面论治选方。其中,肝气郁滞是脏腑功能失调的始发证候,常兼见于便秘的各个证型之中,因此,疏肝解郁方药的应用可贯穿便秘治疗的始终,可在辨证论治基础上加用疏肝解郁药物,对便秘的治疗具有一定的增效作用。同时,治疗时应注意肝体阴而用阳的特性,用药应兼顾疏肝和柔肝之法,重视肝气的条达。。
