摘要

目的探讨双合汤联合硫辛酸注射液治疗糖尿病周围神经病变的疗效及其机制。方法筛选笔者所在医院就诊的糖尿病周围神经病变患者共82例,采取随机数字表法分为对照组和观察组各41例。2组均予常规治疗。对照组给予α-硫辛酸注射液静脉滴注;观察组在对照组基础上给予双合汤治疗。治疗8周后,比较2组痰瘀阻络证症状评分、神经传导速度、临床疗效以及血清同型半胱氨酸（homocysteine, Hcy）、胱抑素C（cystatin-c, Cys-c）、超氧化物歧化酶（superoxide dismutase, SOD）、总抗氧化能力（total antioxidant capacity, TAC）、丙二醛（malondialdehyde, MDA）水平。结果治疗8周后,观察组患者痰瘀阻络证症状评分显著低于对照组,正中神经、腓总神经的运动神经传导速度（motor nerve conduction velocity, MNCV）、感觉神经传导速度（sensory nerve conduction velocity, SNCV）显著高于对照组（P<0.01）;观察组总有效率（90.24%）显著高于对照组（68.29%）（P<0.05）;观察组血清Hcy、Cys-c、MDA水平显著低于对照组,SOD、TAC水平明显高于对照组（P<0.01）。结论双合汤联合硫辛酸注射液通过下调Hcy、Cys-c水平以及氧化应激反应治疗糖尿病周围神经病变痰瘀阻络证,可促进中医证候和神经功能好转,提高临床疗效。
