摘要

劳淋为常见、难治的肾系病证。张炳厚教授认为,劳淋的基本病机是正虚邪伏,病位以肾、膀胱为核心,涉及脾、心、肝、小肠。病性本虚标实,本虚责之于脾、肝、肾气血阴阳亏虚,为发病基础;标实责之于湿热,为发病因素,病理产物为瘀毒,可以互化。治疗上,张教授主张分期辨证论治,急性发作期以邪实为主,治以祛邪为主,强调利水通腑、泻火存阴、解毒散瘀;病情转化期,病性虚实夹杂,治以攻补兼施,强调补中有清,血肉充养,通涩并用,兼顾脾肾;慢性迁延期,以正虚为主,治以扶正为主,强调温固脾肾,阴阳互济;活用补肾八法之清补法,通补法、涩补法、温补法、峻补法、间接补法,同时创制专方,菌毒并治,随证加减,有效降低复发率,提高患者生存质量,疗效确切。
