摘要

难治性眩晕反复发作,严重影响患者的生活质量。病邪久病入络成瘀,痰瘀互结是反复发作、难以治愈的关键。风痰阻络是难治性眩晕发作期的主要病机,发作期治疗以健脾祛湿、化痰息风为主,并关注是否有"新病入络"瘀血与风痰互结的现象,治疗以半夏白术天麻汤为基础,重用天麻并加用钩藤、石决明、丹参、川芎等;缓解期治疗"以通为用",以活血化瘀、化痰通络为主,兼顾培元固本,使血脉气机通畅,临证常用丹参、赤芍、川芎、鳖甲、夏枯草、芒硝等。
