摘要

目的探讨中医药对慢性淋巴细胞性甲状腺炎(CLT)患者发生甲状腺结节(TN)的防治作用。方法收集2014年10月—2017年10月就诊于北京中医药大学东直门医院甲状腺专病门诊的151例慢性淋巴细胞性甲状腺患者资料进行回顾性队列研究,根据服用中药情况分为中药治疗组70例及非中药治疗组81例,统计2组1年内出现TN的情况、中药使用情况。结果中药治疗组TN发生率31.4%,非中药治疗组为49.4%,差异有统计学意义(P<0.05);中药剂量与结节发生之间存在一定的剂量-反应关系,其中<28cDDD、28～90cDDD、91～180cDDD和>180cDDD1年内并发TN率分别为49.4%、33.3%、34.8%和28.9%(P<0.05)。研究涉及中药处方110个,其中性味方面寒、温、平性药及苦、辛、甘味药使用频次较高,归肝经药物使用频次最高,药效方面补虚药、清热药、活血化瘀药、解表药、理气药使用频次较高。结论中医药确有防治CLT患者发生TN作用,治疗符合CLT并发TN的中医病机。

Abstract
