摘要

目的观察针药联合对腹部肥胖型多囊卵巢综合征（PCOS）患者基础体温（BBT）、月经周期情况、性激素、糖代谢及体脂参数的影响。方法将腹部肥胖型PCOS患者随机分为治疗组30例和对照组28例。对照组予中药+饮食运动监督;治疗组在对照组基础上予常规针刺和电针治疗,均连续治疗12周。采用免疫荧光分析法观察治疗前后患者睾酮（T）、雌二醇（E,2,）、促黄体生成素（LH）、卵泡刺激素（FSH）,计算胰岛素抵抗指数（HOMA-IR）,测量体质量（BW）、腰围（WC）、体质指数（BMI）的变化。结果治疗组和对照组的临床总有效率分别为83.33%、57.14%;治疗组E,2,水平高于对照组,T、FSH、LH、HOMA-IR水平均低于对照组（P,<,0.05）;治疗后2组各项肥胖指标均较治疗前改善（P,<,0.05）,且治疗组的改善程度优于对照组（P,<,0.05）。结论针药联合可显著改善腹部肥胖型PCOS患者激素水平,降低BW、WC、BMI及IR状态。

Abstract

Objective To observe the effects of combination of acupuncture and drug on basal body temperature（BBT）,menstrual cycle, sex hormones, glucose metabolism and body fat in patients with abdominal obesity polycystic ovary syndrome（PCOS）.Methods Patients with abdominal obesity PCOS were randomly divided into a treatment group of 30 cases and a control group of 28 cases.The control group was treated with traditional Chinese medicine and diet exercise supervision; the treatment group was treated with acupuncture and electroacupuncture on the basis of the control group, and both two groups was treated continuously for 12 weeks.The immunofluorescence method was used to observe testosterone（T）,estradiol（E2）,luteinizing hormone（LH）,and follicle stimulating hormone（FSH）in the two groups before and after treatment, calculate the insulin resistance index（HOMA-IR）,and measure the body weight（BW）,waist circumference（WC）,body mass index（BMI）.Results The total effective rate of the treatment group and the control group were 83.33% and 57.14% respectively.The level of estradiol in the treatment group was higher than that in the control group, and the levels of testosterone, follicle stimulating hormone and luteinizing hormone and HOMA-IR were lower than those in the control group（P<0.05）.All obesity indicators were improved（P<0.05）,and the improvement effect of the treatment group was significantly better than that of the control group（P<0.05）.Conclusion The combination of acupuncture and medicine can significantly improve hormone levels in patients with abdominal obesity PCOS,and reduce BW,WC,BMI and insulin resistance.
