摘要

子宫内膜异位症属中医学"痛经""癥瘕""不孕"的范畴,其根本病机为瘀血阻滞。赵瑞华教授认为,肝脾不调易致瘀血阻滞,为本病的致病基础。女性易为情志所伤,致肝气郁滞,气郁血瘀,久成癥瘕。正邪斗争胜败的关键在于脾胃的盛衰,脾胃受损则气机升降失调,气机不畅,气血失和,日久成瘀,变生癥瘕。女子以肝为先天,以脾胃为后天之本,肝脾不调,互为因果,治疗需肝脾同调,疏肝健脾与活血化瘀并举。常用小柴胡汤、逍遥散加减疏肝解郁、通达气机,六合定中汤加减补益脾气、健运中州,加丹参、赤芍、鸡内金、皂角刺、三棱、莪术等活血化瘀消癥。对患者进行健康教育,畅达情志、合理饮食、调摄生活,可达治未病的效果。
