摘要

目的基于肺-肠轴理论探讨益气通腑方中药保留灌肠治疗老年慢性阻塞性肺疾病急性加重期（AECOPD）的临床疗效。方法纳入79例老年AECOPD痰热壅肺证兼气虚证患者,随机分为治疗组和对照组。对照组39例予以西医常规综合治疗,治疗组40例在此基础上加用益气通腑方中药保留灌肠,疗程为14 d,对比2组临床疗效及治疗前后粪便球杆比、CD3,+,、CD4,+,、CD8,+,、CD4,+,/CD8,+,等免疫指标的差异。结果治疗组总有效率（97.5%）高于对照组（92.3%）,差异有统计学意义（P,<,0.05）。治疗后治疗组粪便球杆比异常型比例（10%）明显低于对照组（35.90%）,差异有统计学意义（P,<,0.05）。对照组治疗前后CD3,+,、CD4,+,、CD8,+,、CD4,+,/CD8,+,比较,差异均无统计学意义（P,>,0.05）,治疗组治疗后CD4,+,、CD4,+,/CD8,+,高于治疗前,CD8,+,低于治疗前,差异均有统计学意义（P,<,0.05,P,<,0.01）,且治疗组CD4,+,、CD4,+,/CD8,+,均明显高于对照组,差异有统计学意义（P,<,0.05,P,<,0.01）。结论应用益气通腑中药灌肠治疗老年AECOPD疗效确切,可调节肠道菌群,显著改善患者的免疫功能。
