摘要

目的,2,分析混合痔粪便潜血试验（FOBT）阳性患者的结肠镜检查结果和中医证型的关系。,方法,2,收集首都医科大学附属北京中医医院肛肠科自2016年5月—2019年5月1 154例住院行手术治疗的混合痔Ⅲ-Ⅳ期患者，发现139例FOBT阳性，回顾性分析139例患者的辨证分型和结肠镜检查结果。,结果,2,139例FOBT阳性患者中，结肠镜下发现异常病变82例（58.99%），其中结直肠息肉38例，结直肠占位性病变19例，结直肠炎9例，结肠憩室7例，结直肠黑变病6例，直肠溃疡、结肠脂肪瘤、溃疡性结肠炎各1例；结合病理结果，发现139例患者中结直肠良性肿瘤22例（15.83%）、结直肠恶性肿瘤19例（13.67%）。中医证型：湿热下注型71例（51.08%）、气滞血瘀型46例（33.09%）及脾虚气陷型22例（15.83%）。湿热下注型患者多结肠正常，为50例（70.42%）；气滞血瘀型患者以结直肠良性肿瘤居多，为19例（41.30%）；脾虚气陷型患者以结直肠息肉、结肠黑变病居多，均为5例（22.73%）。,结论,2,湿热下注型FOBT阳性混合痔患者临床最多见，且结肠镜检查中往往易发现肠道炎症性疾病；气滞血瘀型FOBT阳性混合痔患者最易发现结直肠良恶性肿瘤；而脾虚气陷型FOBT阳性混合痔患者因疾病日久，累及脾胃，以致多有不同程度的便秘病史。因此，对于FOBT阳性的混合痔患者，结肠镜下发现的结直肠器质性病变与其中医辨证分型存在一定的内在联系，有待进一步研究和挖掘。
