摘要

目的

观察柔行功法治疗良性阵发性位置性眩晕（BPPV）手法复位后残余症状的疗效。

方法

选取2018年6月—2019年12月在北京中医药大学东方医院就诊的BPPV患者，经耳石复位治疗成功后仍有残余症状者160例，按随机数字表法分为对照组、药物治疗组、前庭康复组和柔行功法组，各40例。对照组经耳石复位治疗成功后，未给予其他治疗药物和特殊干预；药物治疗组给予甲磺酸倍他司汀片口服，12 mg/次，3 次/d；前庭康复组采用改良Cawthorne-Cooksey前庭康复训练法，每天早、中、晚各做1次，每次20～25 min；柔行功法组：行柔行功法训练，2 次/d。疗程均2周。观察各组治疗前后残余症状持续时间、眩晕残障量表（DHI）、前庭症状指数量表（VSI）、汉密尔顿焦虑量表（HAMA）评分、眼性前庭诱发肌源性电位（oVEMP）引出率及波形参数的变化。

结果

柔行功法组、前庭康复组和药物治疗组残余症状持续时间

2周的例数均多于对照组（

0.05）；残余症状持续时间

4周的例数少于对照组（

0.05）。各组之间两两比较，残余症状持续时间差异有统计学意义（

0.05）。与治疗前比较，各组治疗后DHI、VSI、HAMA评分均降低（

0.05），柔行功法组各评分下降最明显（

0.05）。各组治疗前后双侧N1、P1潜伏期比较，差异均无统计学意义（

0.05）。与治疗前比较，对照组治疗后oVEMP引出率、不对称比异常率差异均无统计学意义（

0.05）；药物治疗组、前庭康复组和柔行功法组oVEMP引出率升高、不对称比异常率降低，差异均有统计学意义（

0.05），柔行功法组变化最为显著（

0.05）。

结论

柔行功法能更好地缓解BPPV手法复位后残余症状，重建患者的平衡功能，减轻焦虑情绪。
