摘要

出血转化是脑梗死溶栓后的常见并发症之一，症状性出血转化已被明确证实是与脑梗死不良预后密切相关的重要因素，而目前临床尚无明确有效的防治措施。本文从中医理论探讨脑梗死溶栓后出血转化的病机，提出“破瘀耗气、气虚不摄”是其发生的核心病机，离经之血即瘀血，乃脑梗死溶栓后气虚不摄的结果。基于此病机，确立了补气摄血是脑梗死溶栓后出血转化的防治原则，并在该治法的基础上针对离经之瘀血配合应用益气化瘀法，为中医药防治该病变提供新策略。
