摘要

高彦彬教授认为，成人IgA血管炎相关性肾炎（IgAVN）属中医学“络病”范畴，其核心病机责之于虚、热、毒、瘀、风，发病基础为血虚蕴热，斑毒内陷，风伏肾络，瘀血滞络；病性本虚标实，急性期多为虚少实多，迁延期为虚实并重，病势深重时虚多实少；病位以肾络为核心，涉及脾、肝、肺、胃肠、肌肤、关节。治疗上分期辨证论治，攻补兼施。急性期分为风热袭络入营、热毒伤络动血，治以祛邪为主，疏风清热、凉营宁络，以银翘散合清营汤化裁；清热凉血、解毒通络，以犀角地黄汤化裁；迁延期分为肝肾阴虚、肾络瘀滞，气阴两虚、肾络瘀结，脾肾两虚、肾络瘀阻，扶正以益气养阴、滋补肝肾、健脾补肾，祛邪以化瘀通络，分别以知柏地黄丸合二至丸，芪归地黄汤，大补元煎合水陆二仙丹化裁。
