摘要

目的

探讨代谢相关脂肪性肝病（MAFLD）中医证候分布特点及与临床生化参数的相关性。

方法

统计分析210例MAFLD患者的中医证型分布，比较不同证型患者的性别、年龄、体质量、生化指标及肝弹性测定差异。

结果

210例MAFLD患者单一证型比例由高到低依次为湿热蕴结证、肝郁脾虚证、湿浊内停证、痰瘀互结证、脾肾两虚证；性别比较，湿热蕴结证的男性患者比例高于女性患者（

0.05），肝郁脾虚证、痰瘀互结证的女性患者比例高于男性患者（

0.05）；年龄比较，不同证型患者差异无统计学意义（

0.05）；体质量比较，湿热蕴结证、湿浊内停证患者体质量高于其他3种证型（

0.05）。湿热蕴结证患者ALT、GGT均高于其他证型患者（

0.01），湿热蕴结证、湿浊内停证患者TG、TG/HDL-C均高于其他证型患者（

0.01）。轻度脂肪肝占比最高的是肝郁脾虚证、脾肾两虚证，其中肝郁脾虚证和湿热蕴结证、湿浊内停证、痰瘀互结证比较，差异均有统计学意义（

0.05）；痰瘀互结证和脾肾两虚证比较，差异有统计学意义（

0.05）。重度脂肪肝占比最高的是痰瘀互结证和湿热蕴结证，其中湿热蕴结证和肝郁脾虚证比较，痰瘀互结证和肝郁脾虚证、脾肾两虚证比较，差异均有统计学意义（

0.05）。

结论

MAFLD患者主要证候为湿热蕴结证、肝郁脾虚证和湿浊内停证，其中ALT、GGT、TG升高最明显的是湿热蕴结证和湿浊内停证，而TG/HDL-C与MAFLD的发病风险呈正相关，与其最相关的证型亦为湿热蕴结证和湿浊内停证。
