摘要

黄褐斑是一种慢性、获得性面部色素增加性皮肤病，“血瘀”是黄褐斑形成的主要原因。面部为十二经脉气血汇集之处，面部的荣华离不开气血的滋养及经络的运行通畅。经脉作为经络系统主干在内运行气血津液，络脉作为其分支横向沟通内外，运行气血津液，贯通经脉。皮肤血络理论的形成是建立在对皮肤络脉认识基础上，注重皮肤血络、气络、津络相辅相成，认为皮肤气络、津络均依附于皮肤血络，强调皮肤血络是关键枢纽。基于皮肤血络理论对黄褐斑的病因病机和诊疗思路进行探讨，认为痰瘀凝络是其核心病机；外邪伤络、内伤七情、体质易感是其发病原因；病机本虚标实，本为“气络虚”和“血络虚”，实为“血络痰瘀”；中医药治疗可内调外治，内调应审证求因、通补兼施，外治可经皮给药、直达病所，且注重花-藤-虫-胶药物的使用。
