摘要

目的

观察基于病变经络应用温和灸治疗对脑卒中后肩手综合征（SHS）I期患者上肢运动功能的影响。

方法

选择2018年9月—2021年3月北京中医药大学附属护国寺中医医院针灸科收治的98例SHSⅠ期患者，用随机数字表法分为对照组和观察组，各49例。2组均给予药物控制基础病、针灸治疗及常规护理。采用单盲法，2组均在常规治疗基础上使用经络诊察法确定病经后，观察组使用温和灸对病变经络进行治疗，而对照组利用假温和灸对非经非穴区进行治疗。疗程均为6周。对比2组穴位局部皮温、PM

质量浓度、疗效，治疗前、治疗后及治疗结束后8周随访时视觉模拟量表（VAS）评分、上肢Fugl-Meyer评分（FMA）、Barthel指数（BI）评分。

结果

观察组穴位局部皮温高于对照组（

0.05），PM

质量浓度高于对照组（

0.05）。观察组总有效率高于对照组（

0.05）。治疗后、随访时，2组VAS评分均较治疗前降低（

0.05），且观察组VAS评分低于对照组（

0.05）。治疗后、随访时，2组FMA、BI评分均较治疗前升高（

0.05），且观察组FMA、BI评分高于对照组（

0.05）。

结论

基于病变经络应用温和灸治疗可显著改善SHS I期患者疼痛，提升患侧关节运动功能，减轻残疾程度。
