摘要

目的

对原发性胆汁性胆管炎（PBC）患者进行认知障碍（CI）筛查，了解PBC患者与普通人群认知功能的差异，以及PBC中CI发生与体质的关系。

方法

选取上海中医药大学附属龙华医院肝病科就诊的22例PBC患者，同时在社区招募性别、年龄、教育年限、中医体质一一匹配的22例体检健康者作为匹配组（HC组）。运用简易精神状态检查（MMSE）和蒙特利尔认知评估（MoCA），以及全面衰退量表（GDS）、PBC生活质量量表（PBC-40）认知域对PBC相关认知障碍进行筛查。使用功能性近红外光谱技术（fNIRS）测量PBC组言语流畅任务（VFT）期间前额叶及左右颞叶大脑血流动力学变化。使用中医体质量表调查2组人群的体质情况，分析认知障碍与中医体质的关系。

结果

PBC组CI发生率为54.5%，HC组CI发生率为13.6%。PBC组MMSE总分、记忆力、注意力、计算力、回忆能力测试部分均低于HC组，PBC组MoCA总分、视空间与执行、命名、延迟回忆、注意力、语言流畅测试部分低于HC组（

0.05）。PBC组前额叶、左右颞叶积分值均与PBC-40认知域、GDS分级呈负相关，与MMSE总分、MoCA总分、VFT任务期组词总数呈正相关（

0.05）。PBC组虚性体质受试者的认知量表评分、前额叶积分值、左右颞叶积分值、组词数低于HC组虚性体质受试者（

0.05）。

结论

PBC组较HC组更易发生认知障碍，主要表现为记忆力、延迟回忆力、注意力、计算力的下降，VFT任务时fNIRs的前额叶和左右颞叶积分值降低，虚性体质的PBC患者较HC组更易发生认知障碍。
