摘要

目的

构建社区超重/肥胖老年人糖尿病患病风险预测模型，并验证其效能。

方法

通过横断面调查的方式，了解北京市广内社区332例超重/肥胖老年人的糖尿病患病情况，并收集其一般情况、日常生活习惯、合并疾病情况、中医体质类型等信息；采用单因素

Logistic

回归对可能的危险因素进行筛选，进一步行多因素

Logistic

回归分析，并据结果构建社区超重/肥胖老年人糖尿病患病风险预测模型；用受试者工作特征（ROC）曲线检验该模型的预测效能。

结果

选取患有糖尿病的社区超重/肥胖老年人共139例（41.86%）；平均脉压差大（

=1.044）、腰围大（

=1.035）、阳虚质（

=3.529）、湿热质（

=5.544）是社区超重/肥胖老年人患糖尿病的独立危险因素（

0.05），而合并高血压（

=0.286）则具有保护作用（

0.05）。得到公式：logit（

）=1.261×阳虚质（是）+1.713×湿热质（是）+0.043×平均脉压差+0.034×腰围-1.250×高血压（是）-6.086。Hosmer-Lemeshow检验表明该模型预测社区超重/肥胖老年人患糖尿病的机率与实际发生机率比较差异无统计学意义（

0.05），拟合度较好。本次建立的风险预测模型曲线下面积=0.715，95%

为0.658~0.771，说明该预测模型预测效能较高。

结论

根据平均脉压差、腰围、高血压及中医体质成功构建了社区超重/肥胖老年人糖尿病患病风险预测模型，该模型具有较好的预测效能。
