摘要

目的

对比膝关节刃针与膝关节腔内药物注射治疗老年膝骨关节炎的效果。

方法

选取2023年3—9月于首都医科大学附属北京友谊医院疼痛科门诊治疗的老年膝骨关节炎患者，用随机数字表法将患者分为A组、B组，每组40例。2组患者均每天进行规范的股四头肌锻炼，在此基础上A组行膝关节刃针治疗，B组行膝关节腔内注射玻璃酸钠治疗。对比2组治疗前及治疗后4、8、12周的疼痛视觉模拟量表（VAS）评分，膝关节损伤与骨关节炎预后评分（KOOS），塞来昔布累计使用量，不良反应，操作时长、满意度。

结果

2组治疗前及治疗后各时点疼痛VAS评分组间比较，差异无统计学意义（

0.05）；与治疗前比较，2组治疗后各时点VAS评分均降低（

0.05）。2组治疗前及治疗后各时点KOOS组间比较，差异无统计学意义（

0.05）；与治疗前比较，2组治疗后各时点KOOS均升高（

0.05）。治疗后1、4周A组塞来昔布累计使用量高于B组（

0.05），治疗后8、12周2组塞来昔布累计使用量比较差异无统计学意义（

0.05）。2组治疗过程中均未出现严重不良反应。A组出现3例局部肿胀；B组出现2例皮疹、2例剧烈疼痛，均于第2天缓解。2组不良反应发生情况比较差异无统计学意义（

0.05）。A组操作时长短于B组（

0.05），满意度高于B组（

0.05）。

结论

膝关节刃针治疗老年膝骨关节炎的疗效不劣于膝关节腔内药物注射，且属于非药物疗法，患者满意度更高。
