摘要

目的

观察逐水消胀方脐敷对肝硬化腹水患者的疗效。

方法

选取2021年6月1日—12月31日首都医科大学附属北京地坛医院中西医结合中心收治的肝硬化腹水患者70例，采用随机数字表法分为观察组与对照组，各35例。对照组给予规范的基础治疗，观察组给予基础治疗+逐水消胀方脐敷，疗程均为2周。分别于治疗前及治疗1、2周，对比2组中医主要症状积分、腹水深度、腹围、空腹体质量。监测2组不良反应发生情况。

结果

治疗1、2周后，2组患者腹胀、纳差、双下肢水肿、乏力积分均较治疗前下降（

0.05）；治疗1周后观察组腹胀、纳差、双下肢水肿积分均低于对照组（

0.05），治疗2周后观察组腹胀、纳差、双下肢水肿、乏力积分均低于对照组（

0.05）。治疗1、2周后，2组腹水深度、腹围及体质量均较治疗前下降（

0.05），观察组腹水深度及腹围均低于对照组（

0.05）。治疗过程中，观察组有1例患者治疗初期出现脐周局部皮肤潮红，无糜烂、渗出、皮疹、瘙痒等，继续使用未见症状加重。

结论

逐水消胀方脐敷可明显改善肝硬化腹水患者临床症状，且安全性高。

Abstract

Objective

To observe the efficacy of

Zhushui Xiaozhang Formula

in treatment of ascites in patients due to hepatic cirrhosis via umbilical compress.

Methods

Seventy patients with hepatic cirrhosis-induced ascites， who were admitted to the Integrated Traditional Chinese and Western Medicine Center of Beijing Ditan Hospital， Capital Medical University， from June 1 to December 31， 2021， were enrolled and randomly divided into an observation group and a control group according to a random number table， with 35 patients in each group. The control group received standard basic treatment， while the observation group received basic treatment combined with

Zhushui Xiaozhang Formula

via umbilical compress. The treatment duration was 2 weeks. The two groups were compared before treatment and at the 1st and 2nd week of treatment in terms of major traditional Chinese medicine （TCM） symptom scores， ascites depth， abdominal circumference， and fasting body weight. The occurrence of adverse reactions was also monitored.

Results

After 1 and 2 weeks of treatment， the scores for abdominal distension， poor appetite， lower limb edema， and fatigue in both groups decreased significantly compared to pre-treatment levels （

0.05）. After 1 week of treatment， the observation group had lower scores for abdominal distension， poor appetite， and lower limb edema compared to the control group （

0.05）. After 2 weeks of treatment， the observation group had lower scores for abdominal distension， poor appetite， lower limb edema， and fatigue compared to the control group （

0.05）. After 1 and 2 weeks of treatment， both groups showed a decrease in ascites depth， abdominal circumference， and body weight compared to pre-treatment levels （

0.05）. The observation group had significantly lower ascites depth and abdominal circumference compared to the control group （

0.05）. During treatment， one patient in the observation group developed slight redness around the umbilical area at the beginning of treatment， but there was no erosion， exudation， rash， or itching. The symptoms did not worsen after continued treatment.

Conclusion

Zhushui Xiaozhang Formula

via umbilical compress can significantly improve the clinical symptoms of patients with hepatic cirrhosis-induced ascites， with a high safety profile.
