摘要

三焦膜系理论主张全身膜状结构及其附属组织为一个多层次的复杂系统。鼻腔及支气管参与构成小儿外通性上焦膜系结构，与上焦水液代谢密切相关，并通行卫气以司呼吸。变应性鼻炎-哮喘综合征（CARAS）的发生与三焦膜系的分布关系密切。外通性上焦膜系通过间质和胸膜连接肺。外邪扰动，上焦膜系紊乱是CARAS的致病因素；伏饮内停，上焦膜系阻塞是CARAS的病理特征；肺脾肾不足，上焦膜系失养是CARAS的发病基础。亚临床阶段CARAS为外邪侵袭，治疗需疏散外邪，可根据外邪的寒热性质选用苍耳子散、辛夷清肺饮；临床阶段急性期CARAS，饮邪射肺者需祛痰化饮，可根据寒饮及热痰种类选用小青龙汤合苍耳子散或定喘汤合辛夷清肺饮；临床阶段非急性期CARAS为正虚邪恋，治疗需固护正气及祛邪同用，可根据正气虚损的种类使用射干麻黄汤合用玉屏风散、香砂六君子汤、六味地黄丸及金匮肾气丸。
