WANG Yuguang's experience in differentiation and treatment of systemic sclerosis-associated interstitial lung disease from the perspective of"simultaneous treatment of triple-yin exterior and interior"
Professor WANG Yuguang believes that systemic sclerosis-associated interstitial lung disease (SSc-ILD) falls under the traditional Chinese medicine (TCM) categories of "lung impediment" and "lung atrophy". The disease location involves the lungs, spleen, liver, kidneys, and triple energizer, encompassing the five
zang
organs. The etiology is attributed to congenital insufficiency and deficiency of
qi
and blood, with invasion of external pathogens dominated by wind, cold, and dampness. The pathogenesis is characterized by deficiency of
yin
and
yang
of the liver and kidney; invasion of wind, cold, and dampness pathogens into the skin and muscles; impairment
of the nutritive
qi
; stagnation of cold phlegm; involvement of the spleen and stomach's transportation and transformation functions;
qi
stagnation and blockage of collaterals; and progression from impediment syndrome to atrophy syndrome. The clinical treatment principle is to fill the
yin
and
yang
of the liver and kidney, warm and dredge the collaterals, benefit
qi
, and expel pathogens. Treatment focuses on the three
yin
meridians, simultaneously addressing both exterior and interior, effectively alleviating clinical symptoms and improving patients' quality of life.
