摘要

再生障碍性贫血（AA）是由多种因素导致的骨髓造血功能衰竭的血液系统疾病，其发病机制较为复杂（主要与造血微环境、免疫异常有关）。本文以中医学“精血同源”观点为理论基础，分别从肝肾、脾肾，肾阴肾阳的平衡来探讨AA的发病机制，认为“肾虚髓枯”为其基本病机，治疗当以“补肾填精益髓”为基本原则，并根据临床的证候属性予以滋补肝阴、健脾益气治疗，以平衡肾阴与肾阳关系，调整脏腑的功能，为AA的中西医结合临床诊疗提供新的思路与方法。

Abstract

Aplastic anemia （AA） is a hematological disorder caused by bone marrow hematopoietic failure due to multiple factors， and its pathogenesis is relatively complex， mainly involving the hematopoietic microenvironment and immune abnormalities. Based on the traditional Chinese medicine （TCM） theory of "essence and blood homology"， this paper explores the pathogenesis of AA from the perspectives of the balance between liver and kidney， spleen and kidney， and kidney

. It considers "kidney deficiency and marrow exhaustion" as the fundamental pathogenesis. Treatment should be guided by the principle of "tonifying the kidney and replenishing the essence and marrow"， and， according to the clinical syndrome， should also include nourishing liver

and strengthening spleen

to balance kidney

yin

and kidney

yang

and regulate the functions of the

Zangfu organs

. This approach provides new insights and methods for the clinical diagnosis and treatment of AA with integrated TCM and Western medicine.
