摘要

目的

观察柴胡清胰汤灌肠联合生长抑素治疗急性胰腺炎（AP）的临床疗效和安全性。

方法

选择2021年1月—2023年3月定州市人民医院治疗的AP患者100例，采用随机数字表法分为对照组和观察组，各50例。2组均予禁饮食，胃肠减压，纠正水电解质紊乱、营养支持、维持酸碱平衡。对照组给予生长抑素、奥美拉唑钠静脉滴注，1次/d，连续治疗14 d。观察组在对照组基础上，联合柴胡清胰汤灌肠，1次/d，连续治疗14 d。比较2组临床疗效及腹痛缓解、排气、肠鸣音恢复时间、治疗相关不良反应，治疗前后二胺氧化酶（DAO）、C反应蛋白（CRP）、D-乳酸水平。

结果

观察组总有效率高于对照组（

0.05）。观察组腹痛缓解时间、排气时间

、肠鸣音恢复时间均短于对照组（

0.05）。治疗后，2组DAO、CRP、D-乳酸水平均较治疗前降低（

0.05），且观察组低于对照组（

0.05）。2组不良反应发生率比较，差异无统计学意义（

0.05）。

结论

柴胡清胰汤灌肠联合生长抑素治疗AP效果显著，能促进胃肠功能恢复、改善胃肠症状，且未增加不良反应。

Abstract

Objective

To observe the clinical efficacy and safety of

Chaihu Qingyi Decoction

enema combined with somatostatin in the treatment of acute pancreatitis （AP）.

Methods

A total of 100 patients with AP treated at Dingzhou People's Hospital from January 2021 to March 2023 were selected and randomly divided into a control group and an observation group according to a random number table， with 50 cases in each group. Both groups were given fasting， gastrointestinal decompression， correction of water-electrolyte imbalance， nutritional support， and maintenance of acid-base balance. The control group received intravenous somatostatin and omeprazole sodium once daily for 14 consecutive days. On the basis of the control group， the observation group was additionally treated with

Chaihu Qingyi Decoction

enema once daily for 14 consecutive days. The clinical efficacy， time to abdominal pain relief， time to first exhaust， recovery time of bowel sounds， and treatment-related adverse reactions were compared between the two groups. The levels of diamine oxidase （DAO）， C-reactive protein （CRP）， and D-lactate were measured before and after treatment.

Results

The total effective rate in the observation group was significantly higher than that in the control group （

0.05）. The time to abdominal pain relief， time to first exhaust， and recovery time of bowel sounds were all significantly shorter in the observation group than in the control group （

0.05）. After treatment， the levels of DAO， CRP， and D-lactate in both groups were signifi

cantly lower than those before treatment （

0.05）， and the levels in the observation group were significantly lower than those in the control group （

0.05）. The incidence of adverse reactions showed no significant difference between the two groups （

0.05）.

Conclusion

Chaihu Qingyi Decoction

enema combined with somatostatin has a significant therapeutic effect on AP. It can promote the recovery of gastrointestinal function， improve gastrointestinal symptoms， and does not increase adverse reactions.
