摘要

目的

利用马库斯·巴德标尺（MB-Ruler）测量数据观察电针输刺法治疗椎间盘源性腰痛的临床疗效。

方法

选择2023年5月—2024年5月北京中医药大学东直门医院骨科、针灸科就诊的椎间盘源性腰痛患者60例，用随机数字表法分为观察组、对照组，各30例。观察组采用电针输刺腰夹脊穴（L2~L5）治疗，30 min/次，隔天1次，每周3次，连续治疗2周。对照组口服西药塞来昔布胶囊1粒/次，2次/d，连续口服2周。疗程结束后1、3个月进行随访。治疗前后采用MB-Ruler测量腰椎关节活动度，用视觉模拟量表（VAS）评分评价疼痛程度，用Oswestry功能障碍指数（ODI）评价腰部功能障碍程度，统计治疗过程中发生的不良事件。

结果

治疗后，对照组腰椎关节活动度较治疗前无明显变化（

0.05），观察组腰椎关节前屈、后伸、左侧屈、右侧屈活动度均较治疗前增加（

0.05），且优于对照组（

0.05）。治疗后及随访1、3个月，2组VAS、ODI评分均较治疗前降低（

0.05），且观察组VAS、ODI评分均低于对照组（

0.05）。观察组出现皮下血肿2例，均自行缓解。2组均顺利完成治疗，均未发生严重不良反应。

结论

电针输刺法可增加椎间盘源性腰痛患者的腰椎关节活动度，并减轻疼痛、改善腰部功能障碍，且安全性高。

Abstract

Objective

To observe the clinical efficacy of the electroacupuncture infusion method for the treatment of discogenic low back pain based on the MB-Ruler.

Methods

Sixty patients with discogenic low back pain， who visited the Orthopedics and Acupuncture and Moxibustion Departments of Dongzhimen Hospital， Beijing University of Chinese Medicine from May 2023 to May 2024， were randomly divided into an observation group and a control group， with 30 cases in each. The observation group was treated with electroacupuncture at the lumbar

Jiaji

points （L2~L5）， 30 minutes per session， once every other day， 3 times per week， continuously for 2 weeks. The control group received oral celecoxib capsules， 1 capsule per dose， twice daily， for 2 weeks. Follow-up assessments were conducted at 1 and 3 months after the treatment course. Lumbar joint mobility was measured using the MB-Ruler before and after treatment. Pain was evaluated using the visual analogue scale （VAS）， and lumbar functional impairment was assessed using the oswestry disability index （ODI）. Adverse events during treatment were recorded.

Results

After treatment， lumbar joint mobility in the control group showed no significant change compared with baseline （

0.05）， while in the observation group， forward flexion， backward extension， left lateral flexion， and right lateral flexion all increased significantly compared with baseline （

0.05）， and were superior to the control group （

0.05）. After treatment and at 1- and 3-month follow-ups， VAS and ODI scores decreased significantly in both groups compared with baseline （

0.05）， with the observation group showing lower scores than the control group （

0.05）. Two cases of subcutaneous hematoma occurred in the observation group， both resolving spontaneously. All patients in both groups completed the treatment without serious adverse reactions.

Conclusion

The electroacupuncture infusion method can increase lumbar joint mobility， relieve pain， and improve lumbar function in patients with discogenic low back pain， with a high safety profile.
