Interpretation of recommended opinions on Tuina therapy in the diagnosis and treatment standards of knee osteoarthritis in China
The high incidence and high disability rates of knee osteoarthritis (KOA) pose substantial challenges to society, and the disease has become a current research hotspot. With the continuous advancement of standardization, numerous clinical diagnosis and treatment standards for KOA have been developed in China.
Tuina
, including manipulative techniques and traditional exercises, as a traditional external therapy of traditional Chinese medicine (TCM), has unique advantages in the treatment of KOA. This therapy is recommended in most KOA diagnosis and treatment standards; however, there are marked differences in the strength of recommendations and the grading of evidence quality for
Tuina
manipulative techniques and traditional exercises. In the various diagnosis and treatment standards, the strength of recommendations and evidence quality grading for
Tuina
manipulative techniques in KOA are generally low. The reasons include a lack of high-quality randomized controlled trials (RCTs), a shortage of high-quality
systematic reviews and meta-analyses, inconsistencies in reference grading systems for evidence and recommendations, and insufficient standardization in the guideline development process. In contrast, the strength of recommendations and evidence quality grading for traditional exercise therapies in KOA are relatively high across the standards. The reasons include higher quality of the included literature, clearer classification of traditional exercises, and relatively standardized exercise routines.
