摘要

恶性血液病的病因复杂、病情顽固，中医药在改善患者临床症状、减轻放化疗毒副作用、提升生活质量等方面显示出一定作用。历代中医学者认为恶性血液病的发生发展与“毒邪”密切相关。本文系统论述了中医学“毒”理论的多维内涵与外延：药物之 “毒”指药性偏性，通过炮制配伍实现“因毒为能”的治疗作用；病因之“毒”分为外毒（如电离辐射、化学毒物、六淫化毒）和内毒（如痰毒、瘀毒），通过侵袭骨髓或蕴结脏腑等致恶性血液病；病机之“毒”具有迅猛性、危重性、秽浊性及缠绵性等特点，与恶性血液病的临床表现高度契合；病证之“毒”有阴阳毒、髓毒病等；在治疗层面，中医强调以毒攻毒和扶正祛邪原则，采用清热解毒、温化寒毒等治法。通过论述“毒”的理论，并结合现代医学研究，探讨“毒”与恶性血液病之间的关系，旨在为中医药治疗恶性血液病提供新的思路与方向。

Abstract

Malignant hematologic diseases are characterized by complex etiology and refractory course. Traditional Chinese medicine （TCM） has demonstrated unique advantages in this field， and historical TCM practitioners have considered the occurrence and progression of these diseases to be closely associated with "toxin pathogens". This article systematically elaborates on the multidimensional connotations of the TCM concept of "toxin". At the pharmacological level，"toxin" refers to the biased properties of medicinal substances， which can exert therapeutic effects through proper processing and formulation， embodying the principle of "harnessing toxicity for efficacy". At the etiological level， toxins are classified as external toxins （such as ionizing radiation， chemical poisons， and pathological transformations of the six excesses） and internal toxins （such as phlegm-toxin and blood stasis-toxin）， which may cause disease by invading the bone marrow or accumulating in internal organs. At the pathophysiological level， toxin pathogens are characterized by rapid onset， critical severity， filthiness， and chronic entanglement， closely corresponding to the clinical manifestations of malignant hematologic diseases. At the therapeutic level， TCM emphasizes the principles of "fighting toxin with toxin" and "strengthening the body while eliminating pathogenic factors"， adopting treatments such as heat-clearing and detoxification or warming to transform cold toxins. This article aims to provide new perspectives and directions for the treatment of malignant hematologic diseases with TCM， highlighting the unique value of toxin theory in elucidating etiology and pathogenesis and guiding clinical practice.
