摘要

“肾之四最”理论是国医大师柴嵩岩提出的以肾气为核心的辨证思想。卵巢储备功能减退的根本发病机制为肾气亏虚，主要病机为阴血耗伤、血海不充，重要影响因素是肝失调达。在“肾最需护”理论指导下，治疗上重视补肾填精、滋阴养血、平调五脏，同时结合女性不同阶段的年龄特点，临证审查病机，辨证加减，效如桴鼓。

Abstract

The theory of the "four most important characteristics of the kidney

qi"

is a syndrome differentiation concept centered on kidney

， proposed by national TCM master CHAI Songyan. The fundamental cause of diminished ovarian reserve （DOR） is deficiency of kidney

， with the main pathogenesis characterized by depletion of

yin

and blood and insufficiency of the "sea of blood"， while dysregulation of the liver serves as an important contributing factor. Under the guidance of the principle that "the kidney requires the greatest protection"， treatment emphasizes tonifying the kidney and replenishing essence， nourishing

yin

and blood， and regulating the five

Zang

organs in a balanced manner. At the same time， therapeutic strategies are tailored according to the age-related characteristics of women at different stages. In clinical practice， careful assessment of pathogenesis and flexible modification based on syndrome differentiation are applied， yielding rapid and marked therapeutic effects.
