Differentiation and treatment strategies for gastroesophageal reflux disease based on the "triple energizer governing the two axes and the two axes regulating the pivot of qi movement"
Gastroesophageal reflux disease (GERD) is a common digestive system disorder in clinical practice. Based on the interrelationship of the "triple energizer–two axes–
qi
movement", the theory of "triple energizer governing the two axes and the two axes regulating the pivot of qi movement" is proposed to explore the pathogenesis and syndrome differentiation and treatment of GERD in traditional Chinese medicine (TCM). The two axes of the human body refer to the ascending-descending pivot of spleen and stomach
qi
movement, and the entry-exit hub of hepatobiliary
qi
movement. Dysfunction of the triple energizer viscera and disordered
qi
movement of the two axes constitute the initiating factors in the occurrence of GERD.
Qi
stagnation, constrained heat, dampness-turbidity, phlegm-dampness, phlegm-heat, food accumulation, and
qi
counterflow are common syndrome elements of GERD. The basic pathogenesis includes lung congestion with intestinal stagnation, liver constraint with spleen deficiency, upward reversal of stomach
qi
, and turbid pathogens impairing the clear. The therapeutic principles are to
regulate the triple energizer viscera, smooth the
qi
movement of the two axes, unblock stagnation, harmonize the stomach, and descend counterflow. Specific treatment methods include the dredging and tonifying
Yangming
, tonifying the stomach and descending counterflow, clearing
Shaoyang
, clearing the liver and harmonizing the stomach, acrid opening and bitter descending, descending
qi
and resolving phlegm, dispersing and descending lung
qi
, and dispersing the lung and unblocking the bowels. The theory of "triple energizer governing the two axes and the two axes regulating the pivot of
qi
movement" provides theoretical guidance and therapeutic approaches for the treatment of GERD in TCM.
