摘要

目的

观察利胆消石合剂预防肝郁脾虚型胆总管结石（CBDS）患者经内镜逆行胰胆管造影术（ERCP）后复发的效果和安全性。

方法

选取2020年9月—2022年9月北京中医药大学东方医院行ERCP联合内镜下胆道括约肌切开术（EST）和（或）内镜下乳头状球囊扩张术（EPBD）治疗的CBDS患者60例，采用随机数字表法分为观察组、对照组各30例。2组均给予基础治疗，观察组ERCP术后第2天进食后给予利胆消石合剂口服，每周服用5 d（可不连续），疗程6个月。比较2组术后第1、7天及6个月中医证候积分，术后第1、7天AST、ALT水平，术后第6个月或复发时胆总管直径，随访2年内复发率、安全性、疗效。

结果

术后第7天、6个月，观察组中医证候积分低于术后第1天（

0.05），且观察组中医证候积分低于对照组同期（

0.05）。术后第7天、6个月观察组总有效率高于对照组（

0.05）。术后第7天，观察组AST、ALT均降低（

0.05），且低于对照组（

0.05）。术后第6个月，2组胆总管直径（复发患者使用再次行ERCP术中所记录的胆总管直径）较术中缩小（

0.01），且观察组胆总管直径显著小于对照组（

0.01）。观察组复发率低于对照组（

0.05）。所有患者随访期间均未发生明显不良事件，观察组服药期间2次血常规、肝肾功能及心电图检查均未见异常。

结论

利胆消石合剂能够减少ERCP术后肝郁脾虚型CBDS患者复发，并能改善其术后不适症状与体征，保护肝功能、促进胆总管直径恢复，且安全性良好。

Abstract

Objective

To observe the efficacy and safety of

Lidan Xiaoshi

Mixture

in preventing recurrence of liver stagnation and spleen deficiency syndrome-type common bile duct stones （CBDS） after endoscopic retrograde cholangiopancreatography （ERCP）.

Methods

A total of 60 CBDS patients who underwent ERCP combined with endoscopic sphincterotomy （EST） and/or endoscopic papillary balloon dilation （EPBD） at Dongfang Hospital， Beijing University of Chinese Medicine， from September 2020 to September 2022 were enrolled. Patients were assigned to an observation group and a control group using a quasi-random method， with 30 cases in each group. Both groups received routine bas

ic treatment. From the 2nd day after ERCP and after resuming oral intake， the observation group was given oral

Lidan Xiaoshi Mixture

， administered 5 days per week （not necessarily consecutive） for a total course of 6 months. Traditional Chinese medicine （TCM） syndrome score on postoperative days 1 and 7 and at 6 months after surgery， alanine aminotransferase （ALT） and aspartate aminotransferase （AST） levels on postoperative days 1 and 7， and common bile duct diameter at 6 months postoperatively or at recurrence were compared between the two groups. Recurrence rate within 2 years， safety， and overall efficacy were also evaluated.

Results

At 7 days and 6 months after surgery， TCM syndrome scores in the observation group were significantly lower than those at postoperative day 1 （

0.05）， and were also significantly lower than those in the control group at the same time points （

0.05）. The total effective rate in the observation group was higher than that in the control group at both 7 days and 6 months after surgery （

0.05）. At 7 days after surgery， AST and ALT levels in the observation group were significantly decreased （

0.05） and were lower than those in the control group （

0.05）. At 6 months after surgery， the common bile duct diameter in both groups （measured intraoperatively in patients undergoing repeat ERCP due to recurrence） was reduced compared with intraoperative values （

0.01）， and the diameter in the observation group was significantly smaller than that in the control group （

0.01）. The recurrence rate in the observation group was significantly lower than that in the control group （

0.05）. No obvious adverse events occurred during follow-up in either group. In the observation group， repeated blood routine， liver and kidney function， and electrocardiogram examinations during m

edication showed no abnormalities.

Conclusion

Lidan Xiaoshi Mixture

can reduce recurrence of CBDS in patients with liver stagnation and spleen deficiency syndrome after ERCP， improve postoperative symptoms and signs， protect liver function， and promote recovery of common bile duct diameter， with good safety.
