LYU Peiwen's experience in differentiating and treating diabetic foot with "three stages and five types"
The etiology and pathogenesis of diabetic foot in traditional Chinese medicine (TCM) are complex, with highly variable clinical presentations. Professor LYU Peiwen proposes a "three-stage and five-type" system, classifying diabetic foot into non-ulcer stage, ulcer stage, and post-ulcer stage, and combining these with specific syndrome types to achieve integrated disease-syndrome differentiation and precise medication. The integration of internal and external treatment is one of Professor LYU's key clinical characteristics. She advocates the "three methods of tissue regeneration" (removing necrotic tissue, promoting blood circulation, and restoring yang) for internal treatment, combined with external therapies such as poulticing and wound cleansing that act directly on the lesion. Through staged differentiation and dynamic adjustment of treatment strategies, this approach can reduce the level of amputation and improve patients' quality of life, providing a combined TCM-Western medicine clinical pathway for diabetic foot management.
