摘要

心房颤动是以快速无序的颤动波为特点的一种心律失常。近现代医家提倡从“火”论治，但多未明确“火”之病位、病性，对于各脏腑之火对心的影响，以及火之虚实辨析不清，无法直达扼要。临证之中，妄火常与风、痰、瘀等病理产物交织为患，是房颤患者证候复杂、迁延难愈的关键所在。深入辨析妄火之虚实属性、脏腑生火的缘由，厘清其与风、痰、瘀等病理因素的兼夹规律，从而构建以“病性-病位-兼证”为核心的精准辨证体系。

Abstract

Atrial fibrillation is a type of arrhythmia characterized by rapid and disordered fibrillatory waves. Modern traditional Chinese medicine practitioners advocate treating atrial fibrillation from the perspective of "fire"； however， the location and nature of the fire are often not clearly defined， and the effects of fire arising from different Zangfu organs on the heart， as well as the differentiation between deficiency and excess patterns of fire， remain insufficiently clarified， making it difficult to address the essential pathogenesis directly. In clinical practice， delusional fire is often complicated by wind， phlegm， blood stasis， and other pathological factors， which intertwine and contribute to the prolonged and recurrent nature of the disease. This is a key reason for the complexity of syndromes and the chronic， refractory course of atrial fibrillation. Therefore， it is necessary to thoroughly analyze the deficiency-excess nature of delusional fire， explore the causes of fire arising from different Zangfu organs， and clarify its patterns of association with pathological factors such as wind， phlegm， and blood stasis， so as to establish a precise syndrome differentiation system centered on "disease nature-disease location-concurrent syndromes".
