摘要

化生性慢性萎缩性胃炎（MCAG）是一种癌前病变，中医学认为其核心病机为阳化气不足、阴成形太过。张学智教授基于多年临床和理论研究，提出培土畅中法治疗化生性慢性萎缩性胃炎。以“阳化气，阴成形”为切入点挖掘其机制，培土，旨在补益脾胃正气、恢复脾胃运化功能，对应“阳化气”；畅中，通过畅通气机，打破阴邪积聚的病理状态可对应“阴成形”。该治法疗效颇佳，为MCAG的治疗提供了新思路。

Abstract

Metaplastic chronic atrophic gastritis （MCAG） is a precancerous lesion. In traditional Chinese medicine （TCM）， its core pathogenesis is considered to be insufficient yang transforming into

and excessive

yin

forming into substance. Based on years of clinical and theoretical research， Professor ZHANG Xuezhi proposed the method of “strengthening earth and smoothing the middle” for the treatment of MCAG. Taking "

yang

transforming into

and

yin

forming substance" as the conceptual entry point to explore the underlying mechanism， strengthening earth aims to tonify the healthy

of the spleen and stomach and restore their transport and transformation functions， corresponding to "

yang

transforming into

”. Smoothing the middle， by unblocking

movement， addresses the pathological accumulation of

yin

， corresponding to “

yin

forming substance”. This treatment has demonstrated remarkable clinical efficacy and provides a novel approach for the management of MCAG.
