Preliminary exploration of the experience in differentiating and treating Hailey-Hailey disease based on the theory of "healthy qi deficiency and latent pathogens"
Hailey-Hailey disease is a genetically related autoimmune blistering disease characterized by polymorphic skin lesions, including blisters, plaques, erosions, and exudation. This disease commonly occurs in individuals with congenital constitutional weakness and acquired spleen deficiency with excessive dampness, manifesting superficially as insecurity of lung-defensive
qi
and malnourishment of the skin. In the early stage, in
ternal dampness remains latent and hidden without outward manifestation. Later, under the influence of seasonal climatic changes and excessive emotional disturbance, dampness accumulates and transforms into heat, eventually generating toxin, presenting as a "dampness-toxin pattern" characterized by blisters, erosions, and exudation, or a "dampness-retention pattern" characterized by hypertrophy and fissures. In the acute stage, dampness, heat, and toxin are the predominant manifestations and are often accompanied by blood stasis and wind pathogens. Treatment should focus on clearing heat and removing toxin, eliminating dampness and dredging the collaterals, so as to expel pathogenic factors outward. In the stable stage, spleen and kidney deficiency constitute the root pathogenesis, and treatment should focus on strengthening the spleen and replenishing the kidney to promote the distribution of body fluids and prevent recurrence. Treatment should integrate traditional Chinese and Western medicine, emphasize early diagnosis and long-term follow-up, and flexibly apply both internal and external therapies, so as to achieve therapeutic effects that are effective without being overly harsh and that eliminate pathogenic factors without damaging healthy
