摘要:ObjectiveTo observe the clinical effect of Jianpi Yiqi Huoxue Decoction(invigorating spleen qi and activating blood)combined with alendronate sodium on diabetes mellitus complicated with osteoporosis(DOP).MethodsA total of 106 DOP patients admitted to the Department of Endocrinology and Diabetes in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from June 2021 to June 2022 were selected and divided into 2 groups according to random number table method. The control group was treated with alendronate sodium tablets. On this basis, the observation group was treated with Jianpi Yiqi Huoxui Decoction. The clinical effect of 2 groups was observed after 6 months of treatment,including Glucose metabolism indexes [fasting blood glucose(FPG), HbA1c,fasting insulin(FINS), insulin resistance index(HOMA-IR)], bone density, TCM syndrome score, hemorheology indexes [plasma viscosity(PV), whole blood high tangential viscosity(HBV), whole blood low tangential viscosity(LBV), red blood cell aggregation index(EAI)] of the two groups, serum biochemical indexes of bone metabolism, such as alkaline phosphatase(ALP), type I procollagen N-terminal peptide(P1NP), type I collagen cross-linked C-terminal peptide β-degradation product(β-CTX), insulin-like growth factor 1(IGF-1)and Chemerin levels were were compared before and after treatment. The adverse reactions of the 2 groups were analyzed.ResultsThe total effective rate of observation group(88.68%)was higher than that of control group(73.58%)(P<0.05). After treatment, serum levels of FPG, HbA1c, FINS and HOMA-IR in 2 groups were lower than that before treatment(P<0.05). Serum FINS level and HOMA-IR in observation group were lower than those in control group(P<0.05). Bone mineral density (BMD) from L1 to L4 in both groups was increased compared with that before treatment(P<0.05), and TCM syndrome score was decreased compared with that before treatment(P<0.05) and The difference was statistically significant(P<0.05). After the treatment,bone mineral density of femoral neck and total hip in the observation group was higher than that before treatment(P<0.05), but there was no statistical significance in BMD of femoral neck and hip before and after treatment in the control group(P>0.05). PV, HBV, LBV and EAI in the 2 groups were lower than before treatment(P<0.05), and PV, HBV, LBV and EAI in the observation group were lower than those in the control group(P<0.05). Serum ALP, P1NP and β-CTX levels in 2 groups were lower than those before treatment(P<0.05), and serum ALP, P1NP and β-CTX levels in observation group were lower than those in control group(P<0.05). The serum IGF-1 level in 2 groups was higher than before treatment(P<0.05), and the serum Chemerin level was lower than before treatment(P<0.05). The serum IGF-1 level in observation group was higher than that in control group(P<0.05), and the serum Chemerin level was lower than that in control group(P<0.05). In control group, nausea and headache occurred in 1 case each. Nausea occurred in 2 cases and abdominal pain occurred in 1 case in the observation group. There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).ConclusionThe overall curative effect of Jianpi Yiqi Huoxue Decoction combined with alendronate sodium in the treatment of DOP is exact, and its effect may be related to the effective improvement of insulin resistance, correction of hemorheological abnormalities and regulation of serum biochemical indexes of bone metabolism, IGF-1 and Chemerin levels of the integrated traditional Chinese and western medicine program.

摘要:ObjectiveTTo analyze the psychological burden and its correlation with gut microbiota distribution in postoperative patients at early and middle stage of rectal cancer.MethodsA cross-sectional study was conducted, including a total of 44 postoperative patients with early and middle stage rectal cancer from October 2020 to July 2021 with the diagnosis and treatment through Xiyuan Hospital of China Academy of Chinese Medical Sciences and Peking University Cancer Hospital on psychosomatic rehabilitation platform of integrated traditional Chinese and western medicine. Different TCM characteristic factors were divided by TCM syndrome differentiation, Traditional Chinese Medicine Spleen Deficiency Scale (TCM-SDS)and Traditional Chinese Medicine Kidney Deficiency Scale (TCM-KDS). The distribution of postoperative psychological burden and gut microbiota diversity among patients with different TCM characteristics was analyzed and compared. Species differences were compared, and environmental factors were analyzed to determine the influencing factors of different degrees of TCM characteristics and psychological burden on gut microbiota.ResultsAmong 44 patients with early and middle stage colorectal cancer,there were 20 cases of liver depression and spleen deficiency (45.4%), 17 cases of spleen and kidney deficiency (38.6%), 4 cases of phlegm-dampness internal resistance (9.0%), 1 case of qi stagnation and blood stasis (2.2%) and 1 case of liver and kidney deficiency (2.2%). 22 patients with TCM-SDS score > 1.98 were divided into spleen-qi deficiency group and 22 patients with TCM-SDS score ≤1.98 were divided into non-spleen-qi deficiency group. 19 cases with TCM-KDS score > 2.14 were divided into kidney deficiency group, and 25 cases with TCM-KDS score ≤2.14 were divided into non-kidney deficiency group.The scores of self-rating anxiety scale (SAS), self-rating depression scale (SDS) and brief cancer recurrence fear scale (FCRI-SF) in spleen-qi deficiency group were higher than those in non-spleen-qi deficiency group (P<0.05). SAS score, SDS score and FCRI-SF score in kidney deficiency group were higher than those in non-kidney deficiency group (P<0.05).Among 44 patients with early and middle colorectal cancer, there were 24 patients with high anxiety, 20 patients with low anxiety, 31 patients with high depression and 13 patients with low depression.Alpha diversity analysis showed that patients with high anxiety had higher Sobs index (P<0.05). The Sobs index of patients with high depression was lower, but the difference was not statistically significant (P>0.05). The analysis of species differences between groups showed that at the genus level, the abundance of micrococcus rariorum in the intestinal flora of patients with high anxiety level was higher than that of patients with low anxiety level, while the abundance of Veillococcus and Chaetomium was lower than that of patients with low anxiety level (P<0.05). The abundance of Proteus in intestinal flora of patients with high depression was higher than that of patients with low depression (P<0.05).RDA/CCA environmental factor analysis showed that TCM-SDS score was more correlated with the distribution of intestinal flora in patients with high anxiety and depression (P<0.05), and TCM-KDS score was more correlated with the distribution of intestinal flora in patients with low anxiety and depression (P<0.05). The degree of spleen deficiency and kidney deficiency was negatively correlated with the abundance of Bifidobacterium, but positively correlated with the abundance of Bacteroides.ConclusionThe psycho-emotional burden and intestinal flora of patients with early to mid-stage colorectal cancer are associated with the degree of spleen and kidney deficiency.
摘要:ObjectiveTo explore the clinical efficacy of acupoint catgut embedding combined with oral administration of traditional Chinese medicine in the treatment of slow transit constipation of qi deficiency type under the guidance of theory of Three Jiao Viscera.MethodsA total of 63 outpatients admitted to the Beijing Anorectal Hospital(Beijing Erlong Road Hospital)from June 2022 to September 2022 were collected and randomly divided into a control group and an observation group using random number table method. The observation group was treated with acupoint catgut embedding combined with oral administration of traditional Chinese medicine, while the control group was treated with polyethylene glycol 4 000 powder with 10 g/time and 2 times/d combined with mosapride citrate with 5 mg/time and 3 Times/d. Both groups were treated for 8 weeks. Comparative analysis was conducted on chronic constipation severity rating scale(CSS), colonic transit test results, serum NO,Ach, gastrointestinal hormone expression levels SP,MTL and GAS of patients before and after 8 weeks of treatment. The overall effective rate and quality of life (PAC-QOL self-assessment scale) scores of patients with constipation were compared after treatment .ResultsThe total effective rate of the observation group was 90. 62%, which was higher than that of the control group 64. 52%(P<0.05). After 8 weeks of treatment, the frequency of defecation, defecation effort, time to go to the toilet every time, times of unsuccessful defecation and scores of incomplete defecation in the two groups were all lower than those before treatment(P<0.05), and the scores in the observation group were lower than those in the control group(P<0.05), and the amount of markers in the observation group was higher than that in the control group(P<0.05), and the serum levels of Ach, GAS, SP and MTL in the observation group were lower than those in the control group, while the serum levels of Ach, GAS, SP and MTL in the observation group were higher than those in the control group(P<0.05).The quality of life scores of the two groups were higher than before treatment(P<0.05),and the quality of life scores of the observation group were higher than those of the control group(P<0.05).ConclusionAcupoint catgut embedding combined with oral administration of traditional Chinese medicine under the guidance of the Three Jiao Viscera theory can significantly alleviate the symptoms of constipation,increase clinical efficacy,and improve the quality of life of STC type of patients with Qi deficiency.

摘要:Kidney qi imbalance is the main cause of various kidney diseases and abnormal water-liquid metabolism and an important pathogenesis of primary membranous nephropathy (IMN), which was further divided into intrarenal and extrarenal renal qi imbalance. The imbalance of qi inside kidney includes congenital insufficiency of kidney qi and imbalance of qi and blood in kidney. The imbalance of qi outside the kidney also includes the imbalance of opening and closing of kidney qi and the imbalance of ascending and descending of kidney qi.Therefore,the use of the treating disease by balance is a therapeutic concept to correct the deficiency of Yin and Yang in the kidney, so as to calm the kidney qi and restore the relationship between the kidney and other organs, thus making the body function normal. During treatment, attention should be paid to the scale of tonifying kidney and purging kidney, balance the relationship between qi and blood in kidney, and importance attached to the relationship between stomach and kidney, so as to restore the body's own steady state and achieve the functions of relieving diseases and preventing recurrence.

摘要:Myocardial ischemia-reperfusion injury (MIRI) is a condition that poses a significant threat to human well-being. In TCM, MIRI is believed to be rooted in Qi deficiency and characterized by blood stasis. Thus, achieving favorable outcomes requires regulating and treating both Qi deficiency and blood stasis.The polarization state of macrophages in MIRI inflammation exhibits a bidirectional regulation, namely, anti-inflammatory and pro-inflammatory responses, akin to the regulatory implications of Qi and Blood. Based on their correlation, investigating the polarization of macrophages serves to substantiate the relationship between the theory of Qi and blood and MIRI, leading to the identification of the scientific and effective aspects of Qi and blood theory in the treatment of MIRI.

摘要:ObjectiveTo explore the application of Shaoyao Siwu Jieji Decoction after tonsillectomy with adenoidectomy in children with tonsil and adenoid hypertrophy of blood deficiency.MethodsA total of 120 children with blood deficiency syndrome who underwent tonsillectomy with adenoidectomy were randomly divided into Chinese medicine treatment group and western medicine control group according to 1∶1 ratio. The Chinese medicine treatment group was given orally Shaoyao Siwu Jieji Decoction after operation, and the western medicine control group was given intravenous injection of cefuroxime sodium after operation once every 8 hours for one week.The postoperative fever, changes of laboratory indexes before and after operation, facial expression pain scale (FPS-R) score, pseudomembrane in operation area and TCM syndrome score were observed and compared between the two groups.ResultsThe moderate fever rate of the treatment group was lower than that of the control group(P<0.05),The duration of fever days was shorter than that of the control group(P<0.05). After treatment, the white blood cell count (WBC) and C-reactive protein concentration in the treatment group were lower than those in the control group on the first day and third day after operation and the differences were statistically significant (P<0.05), and the differences between the two groups were also statistically significant(P<0.05).After operation, the FPS-R scores of both groups were higher than those before operation (P<0.05), and the observation group was lower than the control group(P<0.05). The time of pseudomembrane shedding in the control group was later than that in the treatment group.After operation, the scores of TCM syndromes in the two groups were lower than those before operation(P<0.05).There was no significant difference between the two groups(P>0.05).The scores of dizziness, fatigue and stool obstruction syndrome in the treatment group were lower than those in the control group, and the difference was statistically significant(P<0.05). There was a statistically significant difference in total score of TCM syndrome score between the two groups (P<0.05).ConclusionThe application of Shaoyao Siwu Jieji Decoction after tonsillectomy and adenoidectomy in children with blood deficiency can reduce or avoid the adverse reactions after tonsillectomy and promote the postoperative rehabilitation of children.
