- 摘要：Through the establishment of TCM evidence-based medicine Center， clinical research registration platform， clinical evidence database and clinical research core index set research center， as well as the standardized development of clinical research， systematic evaluation， guidelines/consensus report， evidence-based medicine plays a major role in explaining the science， effectiveness and safety of TCM. However， the quality of evidence is generally not high enough， research and clinical disjunction and other problems are still facing. In the future， it is necessary to combine evidence-based medicine， narrative medicine and TCM clinical practice， improve the standardization system of evidence-based TCM， carry out evidence-based TCM evidence transformation research and build evidence-based TCM research technology platform in order to further improve the quality of TCM clinical research and promote the integration of TCM with the world.关键词：evidence-based medicine;narrative medicine;traditional Chinese medicine

发布时间：2023-06-25 - 摘要：The concept of core outcome set（COS）was introduced into the field of traditional Chinese medicine（TCM）since 10 years ago.Now nearly 100 COS in the field of TCM have been registered in the database of the Core Outcome Measures in Effectiveness Trials（COMET）and the Registry Platform for Evidence Based TCM.By September 2022，10 COS in the field of TCM had been reported.After analyzing the settings and citation of these COS，the authors found that the citation of the COS in clinical trials and system reviews should be improved，and the value of application for the COS in TCM has not been fully considered.The authors believe that COS has great application value in multiple fields such as original research，secondary research，and transformation research in evidence-based TCM.COS can not only improve the consistency of outcomes reporting in research，improve research value and reduce waste，but also promote the promotion and application of research results.关键词：core outcome sets;evidence-based traditional Chinese medicine;application;methodology

- 摘要：Clinical efficacy is the foundation of the inheritance and development of traditional Chinese medicine （TCM）. The lack of efficacy evaluation methods recognized by both Chinese and western medicine is still an urgent issue to be solved. Comprehensive evaluation method is a kind of method for multiple indicators of a complex system.According to different evaluation purposes， it utilizes various attribute information of the evaluated object and use scientific evaluation methods to conduct an objective， fair and reasonable comprehensive comparison， judgment and sequencing process for the evaluated object. It can combine multiple dimensional outcomes such as western medicine-related outcomes， TCM-related syndromes， and quality of life into a single-dimensional comprehensive index，which can reflect the advantages of the multi-target and multi-dimensional efficacy of TCM， and has been gradually applied to the clinical efficacy of TCM in recent years. This article introduces the principles of several comprehensive evaluation methods commonly used in the field of traditional Chinese medicine， and specific examples of their application in the clinical efficacy evaluation of TCM and matters needing attention. We hope to help TCM researchers to correctly select and use comprehensive evaluation methods in order to provide methodological support for the comprehensive and objective evaluation of the true clinical efficacy of TCM.关键词：Comprehensive evaluation method;randomized controlled trials;traditional Chinese medicine;efficacy evaluation

- 摘要：To introduce a systematic review tool of text and expert opinion （SrTO） tool from Joanna Briggs Institute （JBI）， an evidence-based practice institution was introduced and the inclusion criteria， reference screening， critical appraisal， data extraction， data synthesis and other links integrate the characteristics of traditional Chinese medicine （TCM） experience evidence.SrTO was formed around the five processes of "PICO inclusion criteria"， "Screening strategy"， "data extraction table"， "Quality assessment list"， "Synthesis and rating"， and suggestions were put forward for the systematic review of TCM experience evidence （ancient books， medical cases， expert experience， and so on）.The application of SrTO in the field of TCM may have good methodological research value. It can provide ideas and directions for the evidence synthesis of case report in the field of TCM. The standardization of the evaluation and analysis path of TCM empirical evidence is conducive to improving the utilization of such evidence，and could be used to answer clinical questions in specific situations.关键词：text and expert opinion;systematic review method;TCM;case report;expert experience

- 摘要：ObjectiveTo investigate the component targets and signaling pathways of Jianpi Qinghua Huanyu Formula in the treatment of gastric precancerous lesions （PLGC） based on network pharmacology combined with gene expression database （GEO） chip analysis and molecular docking technology， and to predict its potential mechanism.MethodsThe TCMSP and HERB database was used to collect the effective components and targets of 14 drugs in Jianpi Qinghua Huayu Formula. Differentially expressed genes were obtained through the GSE55696 gene chip in the GEO database. Relevant disease targets of PLGC were obtained by using the DisGeNet， OMIM， and GeneCards disease databases， and the R software was used to obtain the major targets of the treatment of PLGC by Jianpi Qinghua Huayu Decoction. GO and KEGG enrichment analysis of major targets are carried out through R language， and PPI networks of major targets were plotted by STRING database and Cytoscape to screen out core targets， and "effective components - major targets" network of Jianpi Qinghua Huayu Decoction and PLGC were constructed by using Cytoscape to screen out core ingredients components， and finally connect core components with core targets to explore the affinity between proteins and molecules.ResultsA total of 226 effective components of Jianpi Qinghua Huayu Decoction were obtained， corresponding to 416 drug targets. 1673 differentially expressed PLGC genes were obtained， and 1302 related targets were obtained in the disease database， so does 149 intersection genes for drugs and diseases. 2802 items were gained by GO enrichment analysis， and 161 related pathways were obtained through KEGG pathway enrichment analysis， mainly involving AGE-RAGE signaling pathway in diabetic complications and cancer-related signaling pathways. The core effective component quercetin， luteolin， etc. were screened， so do core targets TP53， JUN， AKT1， etc. . Molecular docking technology was used to confirm that there was positive binding activity between core components and core targets.ConclusionJianpi Qinghua Huayu Formula may act on multiple components and targets by participating in multiple signal pathways， thus playing the role of treating PLGC.关键词：Jianpi Qinghua Huayu Formula;precancerous lesions of gastric cancer;network pharmacology;GEO Chip;molecular docking;pharmacodynamic mechanism

发布时间：2023-06-25 - 摘要：ObjectiveTo observe the clinical efficacy of supplementing Qi and Hemostasis Formula in the treatment of the PCI postoperative upper gastrointestinal bleeding in acute coronary syndrome.Methods88 patients with upper gastrointestinal bleeding after PCI from Wangjing Hospital， China Academy of Chinese Medical Sciences， Guang 'anmen Hospital of China Academy of Chinese Medical Sciences and Beijing Chaoyang Hospital affiliated to the Capital Medical University from December 2020 to January 2022 were selected and divided into treatment group of 45 cases and control group of 43 cases according to random number table method. The treatment group was given basic treatment plus Supplementing Qi and Hemostasis Granula，and the control group was given basic treatment plus Supplementing Qi and Hemostasis Granule mock-up. The course of treatment was 7 days. The bleeding rate at 3 days，at 7 days，and within 30 days， the MA value of thrombus elastic graph and the efficacy score of blood stasis evidence in Chinese medicine were observed in both groups.ResultsAfter 7 days of treatment， the bleeding rate in the observation group was lower than that in the control group （P<0.05）. After 7 days of treatment， the MA value in the observation group was decreased compared with that before treatment， while that in the control group increased compared with that before treatment， and the difference between the two groups was statistically significant （P<0.05）.ConclusionBasic treatment combined with supplementing Qi and Hemostasis Formula can effectively treat upper gastrointestinal bleeding after PCI for acute coronary syndrome， significantly improve the TCM blood stasis syndrome of patients，without affecting the antiplatelet activity of clopidogrel， and so it can play a certain antithrombotic effect.关键词：Supplementing Qi and Hemostasis Formula;percutaneous coronary intervention;upper gastrointestinal hemorrhage;clinical research

发布时间：2023-06-25 - 摘要：ObjectiveTo explore the clinical experience of TCM experts in treating ankylosing spondylitis.Methods8 TCM rheumatology experts or their academic successor， including ZHANG Ming-he， YAN Xiao-ping， FENG Xing-hua， TANG Xiao-po， ZHU Wan-hua， WANG Bei， LOU Yu-qian and WANG Yi-jun， were included to conduct semi-structured interviews， record and sort out the interview contents， and adopt ground ed theory to encode and analyze the interview data.ResultsThrough the interview on eight subjects， 20 open codes were excavated. After further deepening the logical relationship， three main shaft codes of diagnosis and treatment of ankylosing spondylitis were obtained， namely， key points of syndrome differentiation， treatment principles and prescription and medication， and the theoretical framework of "clinical experience of famous and old Chinese medicine practitioners in treating ankylosing spondylitis" was initially constructed.ConclusionWhen treating ankylosing spondylitis clinically， famous and old Chinese medicine experts take tonifying kidney and dredging Du meridian， strengthening body resistance and eliminating pathogenic factors， comprehensive methods， treating by stages， applying classics， combining disease differentiation with syndrome differentiation and cooperating with modern test results as the treatment principles. Insect drugs， meridian-guiding drugs， drugs for balancing yin and yang， drugs for protecting stomach qi， drugs for combining diseases and products with feelings of flesh and blood are mostly used in the prescriptions.关键词：Semi-structured interviews;grounded theory;ankylosing spondylitis;experience in TCM;inheritance of expert experience
