      摘要:ObjectiveTo observe the clinical efficacy and safety of ultrasound-guided beryllium needle in the treatment of the third lumbar transverse process syndrome (TLVTPS).MethodsSixty patients with TLVTPS who were treated in Wangjing Hospital of China Academy of Chinese Medical Sciences from February 2021 to February 2022 were selected and divided into ultrasound group and control group using the random number table method, with 30 cases in each group. The ultrasound group was visualized under ultrasound guidance for beryllium needle treatment; the control group was treated with beryllium needle using the empirical pain point manipulation method. Visual analogue scale (VAS) score, lumbar dysfunction index (ODI) score, lumbar mobility in all directions, treatment times and adverse reactions were compared between the two groups before and after treatment.ResultsBefore treatment, there was no significant difference in VAS and ODI scores between the two groups (P>0.05).After 1, 2, 3 and 4 weeks of treatment, the scores of VAS and ODI in both groups decreased (P<0.05),VAS and ODI score in ultrasound group were significantly lower than those in control group (P<0.05).After treatment, the lumbar mobility of the two groups was increased compared with that before treatment (P<0.05), and the lumbar mobility of the ultrasound group in all directions after treatment was greater than that of the control group (P<0.05).In the ultrasound group, there were 1 case, 21 case, 7 case and 1 case after 1, 2, 3 and 4 treatments respectively. There were 0, 6, 17 and 7 cases in the control group treated for 1, 2, 3 and 4 times respectively. The difference between the two groups was statistically significant (P<0.05). Serious adverse events did not occur in both groups.ConclusionUltrasound-guided beryllium needle in the treatment of the third lumbar transverse process syndrome can improve the symptoms of patients with high safety.

      摘要:ObjectiveTo observe the therapeutic effect of self-designed Bushen Quban Decoction on patients with chloasma and explore its possible mechanism of action.Method114 female patients with chloasma who were admitted to the People's Hospital of Kaizhou District, Chongqing from February 2018 to June 2020 were selected. The patients were randomly divided into a control group and an observation group, with 57 patients in each group using a random number table method. The control group received routine western medicine treatment, while the observation group received self-designed Bushen Quban Decoction on the basis of the control group. Both groups had a treatment course of 6 months. The efficacy, syndrome score before and after treatment, serum oxidative stress indicators (superoxide dismutase, malondialdehyde), serum melanotropin, quality of life score, TLRs/MYD88 signal pathway related genes (TLR2, TLR4, MYD88) expression and adverse reactions were compared between the two groups.ResultsThe total effective rate of the observation group was higher than that of the control group (P<0.05). After treatment, the main and secondary syndrome scores of the two groups were decreased compared to before treatment (P<0.05), and the main and secondary syndrome scores of the observation group were lower than those of the control group (P<0.05). After treatment, the level of superoxide dismutase in the two groups was higher than that before treatment, and the level of malondialdehyde was lower than that before treatment (P<0.05), and the level of superoxide dismutase in the observation group was higher than that in the control group, and the level of malondialdehyde was lower than that in the control group (P<0.05). After treatment, the serum levels of melatonin and quality of life in the two groups were lower than before treatment (P<0.05), and the serum levels of melatonin and quality of life in the observation group were lower than those in the control group (P<0.05). After treatment, the expression of TLR2, TLR4, and MYD88 genes in the two groups was lower than before treatment (P<0.05), and the expression of TLR2, TLR4, and MYD88 genes in the observation group was lower than that in the control group (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups(χ2=0.438,P>0.05)。ConclusionSelf formulated Bushen Quban Decoction can improve the therapeutic effect of western medicine on melasma, alleviate oxidative stress response, lower serum melanotropin levels, and improve patient quality of life. Its mechanism of action may be related to down-regulating the expression of TLR2, TLR4, and MYD88 signaling pathway related genes in TLRs/MYD88.

      摘要:ObjectiveTo explore the clinical effect of Tianwang Buxin Dan and Guizhi Gancao Longgu Muli Decoction in treating ventricular premature beats due to deficiency of both qi and yin.MethodsSixty cases of ventricular premature beats who were diagnosed from January 2018 to December 2020 were enrolled, and they were divided into treatment group and control group by random number table. The control group was treated with metoprolol tartrate 12.5 mg or 25 mg each time, twice a day,and the treatment group was treated with Tianwang Buxin Dan and Guizhi Gancao Longgu Muli Decoction on the basis of the control group one bag each day. The course of treatment was four weeks in both groups. The curative effect of premature ventricular contractions, Myerburg grading, curative effect of TCM syndrome, the number of premature ventricular contractions before and after treatment, the related indexes of heart rate variability [standard deviation (SDNN) of all normal RR intervals, root mean square value (rMSSD) of the difference between adjacent RR intervals, the percentage of the number of adjacent RR intervals exceeding 50 ms to the total number of sinus beats (PNN50)] , bigeminal beats and trigeminal beats, paired premature ventricular contractions, TCM syndrome integral and safety were compared between the two groups.ResultsThe total effective rate of premature ventricular contraction in the treatment group was higher than that in the control group (P<0.05). After treatment, the number of premature ventricular contractions in both groups decreased (P<0.05), and the number in the treatment group was less than that in the control group (P<0.05). After treatment, the levels of SDNN, rMSSD and PNN50 in both groups were higher than that before treatment (P<0.05), and the treatment group was better than the control group (P<0.05). After treatment, bigeminal beats and trigeminal beats and the number of paired premature ventricular contractions were decreased compared with that before treatment (P<0.05),in the treatment group, the number of bigeminal beats and trigeminal beats was less than that in the control group (P<0.05).The overall effective rate of Myerburg grade in the treatment group was higher than that in the control group, and the difference was statistically significant (P<0.05). After treatment, the scores of TCM syndrome in both groups were lower than those before treatment (P<0.05), and the treatment group was lower than that of the control group (P<0.05). There were no adverse events in either group.ConclusionTianwang Buxin Dan combined with Guizhi Longgu Muli Decoction are more effective than western medicine used only in the treatment of ventricular premature beats without increasing the incidence of adverse events.

      摘要:ObjectiveTo explore the effects of Timosaponin B-Ⅱ (TB-Ⅱ) on non alcoholic fatty liver disease (NAFLD) and its mechanisms.MethodsIn vitro, HepG2 cells were stimulated with 100 μmol/L palmitate, without or with TB-Ⅱ and the changes of TG level were detected.In vivo, C57BL/6J mice were fed with normal diet group (NCD) , high-fat diet group (HFD), TB-Ⅱ low dose group (HFD+LTB-Ⅱ) and TB-Ⅱ high dose group (HFD+HTB-Ⅱ).The NCD group was fed with ordinary feed, the other groups were fed with high-fat feed for 16 weeks to establish NASH animal models, and the low and high-dose groups of TB-Ⅱ were gavaged with 50 mg/kg and 100 mg/kg respectively every day.The body weight, fat weight, biochemical indexes and liver pathology of mice were monitored to evaluate the degree of liver injury, and the changes of genes related to lipid metabolism in liver tissue were detected by real-time quantitative PCR(RT-PCR).ResultsTG level of HepG2 cellsTB-Ⅱ reduced after TB-Ⅱ treatment, TB-Ⅱ treatment can reduce the body weight of HFD mice,the levels of serum transaminase, TC, TG, LDL-C, TC and TG in liver tissue are also significantly decreased, and the liver pathology is obviously improved. but also downregulated the expression of lipogenesis-related genes (SREBP-1c, FAS and ACC) , accompanied by the up-regulation of the expression of key genes CPT1 and PPARα in fatty acid β oxidation.ConclusionTB-Ⅱ could delay the process of NAFLD induced by high fat by regulating lipid synthesis and fatty acid oxidation in hepatocytes.

      摘要:ObjectiveTo analyze the correlation between TCM syndrome types and test indicators in patients with diabetic foot (DF).MethodsThe medical records of 177 DF inpatients in the Department of Ulcer Vascular Surgery, Beijing Hospital of Traditional Chinese Medicine affiliated to Capital Medical University hospitalized from January 2019 to December 2021 were collected and retrospectively analyzed,and the correlation between each syndrome type and test index was analyzed by Spearman correlation.ResultsAmong the 177 patients, 86 cases (48.6%) had Qi deficiency and blood stasis syndrome, 54 cases (30.5%) had excessive damp-heat syndrome, 37 cases of deficiency of both Qi and Yin, accounting for 20.9%. There was no significant difference in age, sex and the proportion of peripheral vascular disease (PAD), peripheral neuropathy (DFN), diabetic nephropathy (DR) and diabetic retinopathy (DN) among the patients of each syndrome type (P>0.05). There was significant difference in the disease course of type 2 diabetes in patients with different syndromes (P<0.05), and the course of type 2 diabetes in patients with Qi deficiency and blood stasis syndrome was shorter than that in patients with deficiency of both Qi and Yin (P<0.05). There were significant differences in ACE among patients with different syndromes (P<0.05), and the excess of damp-heat syndrome < Qi deficiency and blood stasis syndrome < deficiency of both Qi and Yin, and the ACE of patients with excess of damp-heat syndrome was lower than that of patients with deficiency of both Qi and Yin, with statistical significance (P<0.05). There was no significant difference in LDH-L, CK, CKMB, ALP, BNP, ALT, AST, GGT, TBIL, Cr, UA and UREA among the syndromes (P>0.05). Syndrome types were positively correlated with CK, ACE, Cr and UA (r were 0.155, 0.206, 0.166 and 0.152, respectively, P<0.05).ConclusionAmong DF TCM syndromes,Qi deficiency and blood stasis syndrome is the most common. Patients with excessive dampness and heat syndrome have the lowest ACE level,while patients with deficiency of Qi and Yin syndrome have the highest Cr level.
