      摘要:ObjectiveTo analyze the demographic characteristics, traditional Chinese medicine(TCM)syndromes,and medication patterns of 12 593 gastric cancer patients treated with TCM using portrait technology,and to preliminarily establish patient profiles for gastric cancer.MethodsData from gastric cancer patients treated at Dongzhimen Hospital of Beijing University of Chinese Medicine and Xiyuan Hospital of China Academy of Chinese Medical Sciences from January 1,2019,to December 31,2022,were retrieved from the institutional databases. Information on gender,age,TCM syndrome types,and Chinese medicine formulas was collected.Feature analysis of gastric cancer patients was performed using Python 3.12.0,R 4.2.3,Cytoscape 3.10.0,etc.,to explore medication pattern rules and present different profile characteristics through word clouds,association network graphs,and other forms.ResultsBased on characteristics such as gender,age,TCM syndrome,and core medications,12 593 gastric cancer patients were grouped into 4 clusters based on portrait cluster analysis.The first group consisted of 2 001 cases,with the predominant syndrome type being liver qi invading the stomach (58.6%).The second group had 3 461 cases,and the third group had 5 554 cases,both mainly presenting with phlegm-dampness accumulation syndrome and liver qi invading the stomach syndrome.The fourth group had 1 577 cases,with phlegm-dampness accumulation syndrome accounting for 33.5% and qi stagnation and blood stasis syndrome accounting for 28.7%.High-frequency drugs in the prescriptions included Atractylodis Macrocephalae Rhizoma,Poria,Astragali Radix Praeparata cum Melle/Astragali Radix,Codonopsis Radix,Citri Reticulatae Pericarpium,Angelicae Sinensis Radix,Glycyrrhizae Radix et Rhizoma Praeparata cum Melle/Glycyrrhizae Radix et Rhizoma,Galli Gigerii Endothelium Corneum,Pinelliae Rhizoma,and Amomi Fructus.The total frequency of the four qi categories was 475 times,with cold(36.2%),warm(32.0%),and neutral(21.7%)as the main types.The total frequency of the five flavors was 685 times,with bitter(33.7%),sweet(29.6%),and pungent(24.8%)as the primary tastes.The total frequency of meridian tropisms was 653 times,with the stomach meridian(20.9%),kidney meridian (17.3%),liver meridian (13.5%),and spleen meridian(12.7%)being the most common.High-frequency drugs and their characteristics related to flavors and meridians were consistent across the groups.The association rules for Groups 1,2,3,and 4 were 75,15,26,and 26,respectively.After group screening based on hierarchical tree and heat map results, five core prescriptions were identified.ConclusionUsing portrait technology,12 593 gastric cancer patients treated with TCM were preliminarily categorized into four groups,reflecting their TCM syndromes and medication patterns.

      摘要:ObjectiveTo observe the efficacy of a massage technique primarily based on the“peri-nasal periosteum compression and kneading method”in treating allergic rhinitis children with lung deficiency and cold sensation syndrome.MethodsA total of 60 children with allergic rhinitis who visited Beijing Massage Hospital,Affiliated Huguosi Hospital of Beijing University of Chinese Medicine,Beijing Fengsheng Traditional Chinese Medicine Orthopedic Hospital,and Jinzhan Second Community Health Service Center in Chaoyang District,Beijing,from January 2019 to September 2022 were selected.The patients were randomly divided into two groups,with 30 cases in each group using a random number table method.The control group was treated with montelukast sodium orally,once a day,for 1 week per course,for a total of 4 courses.The observation group received the“peri-nasal periosteum compression and kneading method”in addition to the treatment in the control group,applied every other day,3 times a week,for a total of 4 weeks.The clinical efficacy of the two groups was compared,as well as the scores for the primary symptoms(sneezing,nasal discharge,nasal congestion, and nasal itching)and secondary symptoms(fear of wind and cold,susceptibility to colds,spontaneous sweating,and shortness of breath/fatigue)before and after 3 months of treatment.ResultsThere was no statistically significant difference between the two groups in total effective rate(P>0.05).In the control group,the scores for sneezing,nasal discharge,nasal congestion,nasal itching,and susceptibility to colds decreased at weeks 1,2,3,and 4 compared with those before treatment(P<0.05).At week 4,the fear of wind and cold score decreased(P<0.05).At weeks 3 and 4,the spontaneous sweating score decreased compared with those before treatment(P<0.05).There was no significant difference in shortness of breath/fatigue scores at any time point(P>0.05).In the observation group,the scores for sneezing,nasal discharge,nasal congestion,nasal itching,fear of wind and cold,susceptibility to colds,and spontaneous sweating decreased at weeks 1,2,3,and 4 compared with those before treatment(P<0.05).At weeks 2,3,and 4,the scores for shortness of breath/fatigue also decreased compared with those before treatment(P<0.05).There were no significant differences in primary symptom scores between the two groups before treatment and at week 1(P>0.05).At week 2,the observation group had significantly lower scores for sneezing,nasal congestion,and nasal itching compared with the control group(P<0.05).At week 3,the observation group had lower scores for all primary symptoms compared with the control group(P<0.05).At week 4,the observation group had significantly lower scores for sneezing,nasal discharge,and nasal congestion compared with the control group(P<0.05).There were no significant differences in secondary symptom scores between the two groups at any time point(P>0.05).Three months after treatment,the symptom scores in both groups were lower than those before treatment(P<0.05),and the observation group had significantly lower scores for sneezing,nasal discharge,nasal congestion,nasal itching,and spontaneous sweating than the control group(P<0.05).ConclusionThe“peri-nasal periosteum compression and kneading method”can improve the clinical efficacy and alleviate symptoms in allergic rhinitis children with lung deficiency and cold sensation syndrome.

      摘要:ObjectiveTo observe the clinical efficacy of modified Buyang Huanwu Decoction combined with anticoagulant therapy in the early treatment of acute stage of deep vein thrombosis (DVT) in the lower limbs.MethodsA total of 45 patients with acute lower limb DVT treated at the Vascular Surgery Department of Wangjing Hospital, China Academy of Chinese Medical Sciences, from October 2021 to July 2023 were randomly divided into an observation group (n=23) and a control group (n=22) using a random number table method. The observation group received oral modified Buyang Huanwu Decoction granules combined with basic anticoagulant therapy, while the control group received basic anticoagulant therapy alone. The treatment course lasted for 3 months. The Villalta score, bilateral leg circumference differences, coagulation function indicators [serum D-dimer(D-D), prothrombin time (PT), and activated partial thromboplastin time (APTT)] at baseline (before treatment) and at 1,3, and 6 months after treatment, and lower limb venous recanalization at 6 months were compared between the two groups.ResultsAfter 1 month of treatment, there were no statistically significant differences between the two groups in terms of symptoms, signs, or total Villalta score(P>0.05). After 3 and 6 months of treatment, the symptoms, signs, and total Villalta scores of both groups had decreased compared with those before treatment, with the observation group showing significantly lower scores than the control group(P<0.05). At 1,3, and 6 months after treatment, the thigh and calf circumference differences in both groups were reduced compared with those before treatment, with the observation group showing significantly greater reductions than the control group(P<0.05). After 1,3, and 6 months of treatment, serum D-D levels in both groups were reduced, while APTT and PT were prolonged compared with those before treatment, with the observation group showing significantly better results than the control group(P<0.05). The complete recanalization rate in the observation group was significantly higher than that in the control group (χ2=1.463,P<0.05).ConclusionModifed Buyang Huanwu Decoction combined with early anticoagulant therapy can significantly improve the clinical symptoms of patients with PTS of lower limb DVT, and its efficacy is superior to anticoagulant therapy alone.

      摘要:ObjectiveTo explore the effect of Shenyuan Yiqi Huoxue Capsules(SYD)on myocardial injury and coronary microcirculation in patients with unstable angina(UA)following percutaneous coronary intervention(PCI).MethodsA total of 68 patients with coronary heart disease(CHD)and UA who were treated in the Department of Vasculocardiology at Beijing Hospital of Traditional Chinese Medicine,Capital Medical University from June 2019 to June 2023 were enrolled and randomly divided into an observation group and a control group according to a random number table,with 34 patients in each group.The observation group received SYD orally in addition to standardized treatment,while the control group received placebo(PBO)orally in addition to standardized treatment.Both groups underwent PCI.The myocardial injury markers, including creatine kinase-MB(CK-MB),CK-MB peak values,coronary microcirculation index(IMR),and the change rate of IMR(△IMR)in both groups,were compared before and after treatment,along with safety evaluations.The correlation between CK-MB peak values and △IMR was analyzed using Spearman's correlation method.ResultsTwenty-four hours after surgery,the CK-MB levels in both groups were higher than those before surgery(P<0.05),and the observation group had lower CK-MB levels than the control group(P<0.05).At 72 hours post-surgery,the CK-MB levels in both groups showed no significant difference compared with pre-treatment values(P>0.05).The CK-MB levels in the observation group were lower than in the control group,but the difference was not statistically significant(P>0.05).The peak CK-MB value in the observation group was lower than in the control group(P<0.05).After surgery,the IMR levels in both groups were higher than those before surgery(P<0.05),but the difference between the two groups was not statistically significant(P>0.05).The △IMR value in the observation group was lower than in the control group(P<0.05).There was no significant correlation between CK-MB peak value and △IMR in the control group(r=-0.158 5,P>0.05),while in the observation group,a positive correlation was found between CK-MB peak value and △IMR(r=0.339 2,P<0.05).In the observation group,one patient had poor lipid control and developed elevated alanine aminotransferase(ALT)after doubling the dose of atorvastatin,which returned to normal after stopping the medication.No abnormalities in liver or kidney function,routine blood/urine tests,or coagulation indicators were observed in the remaining patients.No drug-related dermatitis,bleeding,or other adverse reactions were observed in either group.ConclusionSYD can improve myocardial injury and coronary microcirculation in patients with UA after PCI,with high safety.

      摘要:ObjectiveTo investigate the effects of Fuyou Mixture on the secretion of gonadotropin-releasing hormone(GnRH)in the hypothalamus of female rats with precocious puberty through the phosphoinositide 3-kinase-protein kinase B(PI3K/Akt)signaling pathway.MethodsA total of 75 female SD rats were randomly divided into a normal group,a model group,a Fuyou Mixture group,a leuprolide group,and a quercetin group according to a random number table,with 15 rats in each group.The rats were fed breast milk until 23 days of age.On day 5,except for the normal group,the other four groups received a single subcutaneous injection of 25 μL of danazol solution in the neck to establish the precocious puberty model.Successful modeling was confirmed by an earlier vaginal opening time, earlier first estrus, and abnormal estrous cycles. After 10 days, the leuprolide group received subcutaneous injections of leuprolide(100 μg/kg)on the left lower abdomen, the Fuyou Mixure group received Fuyou Mixture at 6 mL/kg by gavage daily,and the quercetin group received quercetin at 4 mg/kg by gavage daily. The normal and model groups received normal saline at 2 mL/kg by gavage.Vaginal opening was checked starting from day 20,and ovarian development was observed using hematoxylin-eosin (HE) staining.Serum levels of luteinizing hormone(LH),follicle-stimulating hormone(FSH),and estradiol(E2)were measured by enzyme-linked immunosorbent assay(ELISA).Immunohistochemistry was used to detect GnRH protein expression in the hypothalamus.Real-time quantitative polymerase chain reaction(RT-qPCR)was used to detect mRNA expression of GnRH, Kiss-1, G protein-coupled receptor 54(GPR54),insulin-like growth factor(IGF-1),and estrogen receptor α(ESR1)in the hypothalamus. Western blotting was used to assess the protein expression of GnRH,PI3K,phosphorylated PI3K(p-PI3K),Akt,and phosphorylated Akt(p-Akt).ResultsCompared with the normal group, the model group showed an earlier vaginal opening.The model group had higher uterine and ovarian coefficients than the normal group(P<0.05).The Fuyou Mixture group had a smaller uterine coefficient than the model group(P<0.01), and the leuprolide group showed smaller uterine and ovarian coefficients than the model group(P<0.05).No significant pathological changes were observed in the morphology of ovarian and uterine cells among the model,Fuyou Mixture,leuprolide,and quercetin groups,indicating that danazol did not cause pathological growth of the sexual organs. Ovarian development in all groups showed primary,secondary,and mature follicles.There was no significant difference in the uterine wall thickness between the model group and the normal group(P>0.05).The Fuyou Mixture group showed a smaller uterine wall thickness than the model group,but the difference was not statistically significant(P>0.05),while the leuprolide group had a significantly thinner uterine wall than the model group(P<0.05).No significant differences in uterine wall thickness were observed between the quercetin group and the model group(P>0.05).The serum levels of E2,LH,and FSH were significantly higher in the model group than in the normal group(P<0.05).E2 and FSH levels were lower in the leuprolide group than in the model group(P<0.05),and both the Fuyou Mixture and quercetin groups showed significantly lower serum E2 levels than the model group(P< 0.01).The average optical density of GnRH cells in the hypothalamus was higher in the model group than in the normal group(P<0.05).The Fuyou Mixture and leuprolide groups showed lower optical density values than the model group(P<0.01),while the quercetin group showed no significant difference from the model group(P>0.05).Under the microscope,GnRH-positive cells with varying shades of brown-yellow were observed in all groups,with clear cell structure and prominent nucleoli,most concentrated in the Broca's diagonal band.The mRNA expression levels of GnRH,Kiss-1,GPR54,ESR1,and IGF-1 in the hypothalamus were significantly higher in the model group than in the normal group(P<0.01).The Fuyou Mixture and leuprolide groups showed lower mRNA expression of GnRH, Kiss-1, GPR54, ESR1, and IGF-1 than the model group(P<0.05),while the quercetin group showed lower mRNA expression of GnRH,Kiss-1,and ESR1 than the model group(P<0.05).The relative protein expression of GnRH, p-Akt/Akt, and p-PI3K/PI3K in the hypothalamus was significantly higher in the model group than in the normal group(P<0.01).The Fuyou Mixture, leuprolide, and quercetin groups showed lower protein expression of GnRH, p-Akt/Akt, and p-PI3K/PI3K than the model group(P<0.05).ConclusionFuyou Mixture exerts therapeutic effects on precocious puberty in rats by downregulating IGF-1 mRNA expression and the protein expression of p-Akt/Akt and p-PI3K/PI3K and inhibiting the IGF-1/PI3K/Akt signaling pathway to reduce the expression of Kiss-1 and GPR54 mRNA,thereby decreasing GnRH secretion and serum E2 levels and inhibiting the activation of the hypothalamic-pituitary-gonadal axis.

      摘要:ObjectiveTo investigate the effects and mechanisms of Psoralea corylifolia extract on chemokine ligand 4 (CCL4), inflammatory response, and bone metabolism in osteoporosis model rats.MethodsA total of 55 SPF-grade female SD rats were selected, with 10 rats assigned to the blank group. The remaining 45 rats were used to establish an osteoporosis model. Forty successfully modeled rats were divided into the model group and low, medium, and high dose groups of P. corylifolia extract, with 10 rats in each group. On the 9th week of modeling, the low, medium, and high dose groups were administered P. corylifolia extract at doses of 50, 100, and 200 mg/(kg•d) by gavage, respectively. The blank and model groups were given physiological saline at 10 mL/(kg·d). The intervention lasted for 8 weeks. After 8 weeks of intervention, the rats were euthanized, and femur tissue was collected. Histopathological changes in the femur were observed under a light microscope after hematoxylin-eosin (HE) staining. CCL4 and inflammatory cytokines [tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6)]were detected by enzyme-linked immunosorbent assay(ELISA).Bone metabolism markers[osteocalcin(OC),type I collagen N-terminal propeptide (P1NP)]were detected by electrochemiluminescence. Western blotting was used to detect the expression of transforming growth factor-beta 1(TGF-β1)/Smad4 signaling pathway-related proteins in the femur tissue.ResultsThe bone marrow cavity area was smallest in the blank group and largest in the model group. The bone marrow cavity area in the low, medium, and high-dose P. corylifolia extract groups gradually decreased, but remained larger than that in the blank group. Compared to the model group, the CCL4 level in the low, medium, and high-dose groups was significantly lower (P < 0.05), and the level of CCL4 decreased in a dose-dependent manner (P < 0.05). The levels of OC and P1NP were lower in the model than in the blank group (P < 0.05). The OC and P1NP levels in the low, medium, and high-dose P. corylifolia extract groups were higher than that in the model group (P < 0.05), with a dose-dependent increase observed (P < 0.05). The levels of TNF-α and IL-6 were higher in the model group than in the blank group (P < 0.05). The levels of TNF-α and IL-6 in the low, medium, and high-dose groups were significantly lower than those in the model group (P < 0.05), with a dose-dependent decrease (P < 0.05). The relative protein expression levels of TGF-β1 and Smad4 in the model group were lower than those in the blank group (P < 0.05). In contrast, the relative protein expression levels of TGF-β1 and Smad4 in the low, medium, and high-dose P. corylifolia extract groups were significantly higher than those in the model group (P < 0.05), with a dose-dependent increase (P < 0.05).ConclusionP. corylifolia extract can reduce the CCL4 level in osteoporotic rats, suppress inflammatory response, and improve bone metabolism. The underlying mechanism may be related to the inhibition of the TGF-β1/Smad4 signaling pathway.

      摘要:Diabetic macular edema (DME) is a common ocular complication of diabetes, referred to as“eye disease in consumptive thirst”in traditional Chinese medicine (TCM), which is primarily caused by yang deficiency in the lungs, spleen, and kidneys, leading to an inability to regulate water, resulting in its accumulation in the macula. This condition can significantly affect a patient's vision. According to Huangdi Neijing(Su Wen: Tang Ye Lao Li Lun), the source of the disease is attributed to the weakness of the yang qi of the five organs, which causes the accumulation of body fluids, leading to edema. For edema, it proposes three methods of water treatment, i.e., opening sweat pores, cleaning the bladder, and eliminating stagnated water. In the case of DME, which is considered a form of ocular edema, these three methods can be applied to warm yang, excrete water, promote water metabolism, expel the pathogenic water, smooth the flow of Yangqi, and achieve the effects of regulating Qi and blood, eliminating water retention, and improving vision.
