      摘要:ObjectiveTo analyze the biological basis of qi and yin deficiency syndrome in type 2 diabetes by using proteomics and metabolomics technology.MethodsFrom May 2018 to September 2018, 30 cases of type 2 diabetes with qi-yin deficiency syndrome (group Q), 30 cases of type 2 diabetes with non qi-yin deficiency syndrome (group F) and 30 cases of healthy control group (group N) were enrolled in Beijing Shijitan Hospital affiliated to the Capital Medical University. Label free proteomics method was used to analyze the differential proteins of three groups of plasma samples,and the differential expression proteins between groups were obtained. KEGG pathway and String database were used to analyze the bioinformatics of the differential expression proteins. LC-MS combined technology was used to analyze differential metabolites in three groups of plasma samples. A combination of multidimensional analysis and single dimensional analysis was used to screen differential metabolites among the three groups. The HMDB database was searched and identified, and KEGG path analysis was performed on the screened differential metabolites to explore the metabolic pathways in which differential metabolites may participate.ResultsThe results of plasma proteomics were that 119 differential proteins were screened between the type 2 diabetes group and the healthy control group,of which 55 were up-regulated and 64 were down-regulated;89 differential proteins were screened out between type 2 diabetes with qi-yin deficiency syndrome and non qi-yin deficiency syndrome, among which 51 were up-regulated and 38 were down-regulated;18 differentially expressed proteins were screened out between the type 2 diabetes group with qi and yin deficiency syndrome,the non qi and yin deficiency syndrome group and the healthy control group,of which 8 were up-regulated and 10 were down-regulated. KEGG pathway analysis found that PI3K-AKT signaling pathway, renin secretion, IL-17 and other signaling pathways were closely related to the occurrence and development of qi-yin deficiency syndrome in type 2 diabetes. The results of plasma metabonomics were that 32 differential metabolites were screened out between the type 2 diabetes group and the healthy control group. KEGG pathway analysis found that branched chain amino acid (BCAAs)synthesis and metabolic pathway, glycerol phospholipid metabolism, alanine, aspartic acid and glutamic acid metabolic pathway may be closely related to the occurrence and development of type 2 diabetes. Further integration of proteomics and metabolomics results found that BCAAs were closely related to PI3K-AKT signaling pathway.ConclusionHigh levels of BCAAs in plasma may cause damage to insulin signaling pathway by inhibiting PI3K-AKT pathway, leading to the occurrence and development of Qi and Yin deficiency syndrome in type 2 diabetes.

      摘要:ObjectiveTo investigate the curative effect of holistic stratified needle-knife releasecombined with bone injury massage on lumbar disc herniation (LDH).MethodsA total of 380 LDH patients diagnosed and treated from March 2019 to March 2021 in Guangdong Integrated Traditional Chinese and Western Medicine Hospital were selected as the study objects. According to the random coin toss method, they were divided into the observation group (n=192), who were given the holistic stratified needle-knife release combined with bone injury massage once a day with 4 weeks as one course, and the control group (n=188), who were given the bone injury massage as same as the observation group. On the 1st, 5th and 9th day, the whole layered needle-knife release was used. Lumbar motion, TCM syndrome score and visual analog scale (VAS) score were compared before and after treatment.ResultsThe excellent and good rate of the observation group was higher than that of the control group (P<0.05). After treatment, the flexion and extension of the lumbar spine in both groups were increased (P<0.05), and the flexion and extension of the lumbar spine in the observation group were higher than those in the control group (P<0.05). After treatment, the total scores of TCM syndromes in the two groups were lower than those before treatment (P<0.05), and the total scores of TCM syndromes in the observation group were lower than those in the control group (P<0.05). After treatment, the VAS scores of the two groups were lower than those before treatment (P<0.05), and the VAS scores of the observation group were lower than those of the control group (P<0.05).ConclusionThe holistic stratified needle-knife release combined with bone trauma massage has a significant effect in the treatment of LDH, and it can significantly improve lumbar function and clinical symptoms, and relieve pain.

      摘要:ObjectiveTo explore the expansion of the inspection index by taking the syndrome of qi deficiency and blood stasis as the representative, and provide the basis for the feasibility of syndrome diagnosis of model animals.MethodsA total of 30 mice were randomly assigned to the blank control group,qi deficiency and blood stasis group,and the herbal counterfactual group, with 10 mice in each group. The mice in both qi deficiency and blood stasis group and the herbal counterfactual group were subjected to a modeling protocol involving starvation, fatigue, exposure to cold and dampness for a duration of 20 days. In the herbal counterfactual group, the mice received a daily oral gavage of concentrated Astragalus-Curcuma decoction, starting from the third day of modeling. Conversely, the mice in the blank control group and the qi deficiency and blood stasis group were gavaged with the same volume of distilled water daily for 18 days. The general condition and body mass of the mice was observed continuously. At the end of the modeling period, the water content of feces and appetite for food were assessed. Additionally, the behavioral characteristics of the mice in each group were evaluated using a wind avoidance test, mechanical pain field assessment, and open field test.ResultsCompared with the blank control group, the body weight, the staying time in the 2nd and 3rd zones in the shelter experiment, the total distance, the average speed, the times of crossing the grid in the open field experiment, the mechanical pain zone and the R, G and B values of the tongue color of the mice in the Qi deficiency and blood stasis groups and the Chinese medicine counter-evidence group were all lower (P<0.05,P<0.01). Compared with the group of qi deficiency and blood stasis, the body weight, the staying time in the second and third regions in the shelter experiment, the total distance, the average speed, the times of crossing the grid in the open field experiment and the R,G,B values of tongue color were all higher in the group of TCM (P<0.05,P<0.01).ConclusionThe indexes of shelter from the wind and mechanical pain field mice, as well as the indexes of open shelter experiment, fecal moisture content and tongue color detection, can be used as extended inspection indexes for diagnosing qi deficiency and blood stasis syndrome in mice, but foraging desire cannot be used as an effective index.

      摘要:ObjectiveTo investigate the effect of artesunate combined with levofloxacin on the antibacterial activity of carbapenem-resistant Escherichia coli(CRECO).Methods20 strains of CRECO isolated from the laboratory were collected and the minimum inhibitory concentrations (MIC) of artesunate, levofloxacin, ceftazidime and imipenem were determined by broth microdilution methods. The synergistic antibacterial activity of artesunate with levofloxacin, ceftazidime and imipenem was determined by broth microdilution checkerboard methods. The effects of different treatment groups on the growth of clinical strain (E5)of CRECO were observed by bacterial growth curve method. The expression of efflux pump gene and regulatory gene was detected by RT-PCR.ResultsThe MIC50 of artesunate, levofloxacin, ceftazidimeandimipenem were >8 192,64,2 048 and 32 µg/mL, respectively. Synergistic antibacterial test showed that when artesunate was combined with levofloxacin at 1 024 µg/ mL, the MIC50 decreased from 64 µg/mL to 16 µg/mL (i.e.,1/4 MIC50), and the combined effect was mainly synergistic. When combined with ceftazidime, its MIC50 decreased from 2 048 µg/mL to 1 024 µg/mL (i.e.,1/2 MIC50), and the combined effect was mainly additive. The results of the growth curve showed that artesunate combined with levofloxacin could effectively inhibit the growth of CRECO, and the curve tended to be flat. All 20 strains carried the efflux pump genes, AcrA and AcrB genes. RT-PCR results showed that artesunate combined with levofloxacin could reduce the expression of efflux pump gene AcrB. The decrease of its expression level was associated with the decreased expression of the regulatory gene AcrB.ConclusionArtesunate has no antibacterial activity against CRECO, but artesunate combined with levofloxacin could effectively enhance the antibacterial action of antibiotics, and its synergistic inhibitory mechanism may be related to the reduction of the expression of efflux pump gene AcrB.

      摘要:Refractory gastroesophageal reflux disease (RGERD) is a common digestive system disease with recurrent reflux symptoms after proton pump inhibitor (PPI) treatment. It is mainly caused by the reflux of gastric contents into the esophagus, mouth or lung. It is believed that the imbalance of qi,fire and body fluid caused by the unfavorable Shaoyang cardinal is the key to the pathogenesis of this disease. In the treatment,Shaoyang gallbladder and triple energizer should be considered in order to reconcile the pivot and Xiaochaihu Decoction and Haoqin Qingdan Decoction used as the main prescription and modified with the syndrome with significant clinical efficacy.
