      摘要:The morbidity of metabolic dysfunction-associated steatotic liver disease (MASLD) is the highest among liver diseases, and the combination of autoimmune hepatitis (AIH) and MASLD is commonly seen clinically. Although the clinical manifestations of these two liver diseases are similar, there are significant differences in pathologic mechanism and diagnosis and treatment, and there is a lack of consensus on the research and diagnosis and treatment of these two liver diseases because of the complexity of the condition and the difficulty of the treatment after the concurrency. This paper firstly analyzes the existing difficulties in diagnosis and treatment of AIH combined with MASLD, summarizes the main identification points of the two, and initially put forward the clinical diagnosis and treatment flowchart; then, it points out that AIH combined with MASLD belongs to the category of combined diseases in Chinese medicine, and the main pathogenesis is depression of liver, deficiency of spleen and kidney, obstruction of dampness, heat, phlegm, stasis and blood stagnation, and the basic treatment principles are to dredge liver, strengthen the spleen and benefit the kidney, and to clear away heat and resolve phlegm and activate blood circulation. Codonopsis, Poria, Atractylodes macrocephala, Ligustri lucidi fructus, Lycii fructus, Chuanxiong rhizome, Paeoniae radix rubra, Scutellariae radix, Coicis semen, Citri reticulatae pericarpium, Pinelliae rhizome, Fritillariae thunbergii bulbus are used as the basic formula, and it is tailored to the characteristics of the individual's physique and the course of the disease, and can be flexibly combined with the formula to improve the clinical symptoms and the patient's prognosis. Finally, it is suggested that in the clinical diagnosis and treatment,AIH combined with MASLD should be emphasized, and the clinical research on diagnosis and treatment on AIH combined with MASLD should be actively carried out so as to improve patients' prognosis.

      摘要:ObjectiveTo investigate the clinical effect of Fuyuan Huoxue Decoction combined with percutaneous kyphoplasty (PKP) in the treatment of thoracolumbar compression fractures.MethodsA total of 64 patients with thoracolumbar vertebral compression fractures, who visited Department of Orthopedics, Beijing Hospital of Traditional Chinese Medicine, Capital Medical University from November 2020 to November 2021, were divided into experimental group and control group by block randomization method. 32 patients in the control group were treated with PKP surgery, and 32 in the experimental group was treated with Fuyuan Huoxue Decoction orally for one week on the basis of the control group. The clinical efficacy, visual analogue scale (VAS), Oswestry dysfunction index (ODI) score, TCM syndrome score, the height of anterior and midline of injured vertebra were compared between the two groups.ResultsThe effective rate of the experimental group was 25.00% (8/32), which was significantly higher than that of the control group, 0.00% (0/32), and the difference was statistically significant(P<0.05). There was no significant difference in the total effective rate between the two groups(P>0.05).After one week of treatment, VAS and ODI scores of the two groups of patients were significantly lower than those before treatment(P<0.05), and the scores of the experimental group were significantly lower than those of the control group, and the difference was significant(P<0.05). After treatment,the scores of TCM syndromes in both groups were lowered than those before treatment(P<0.05), and the scores of TCM syndromes in the observation group were lowered than those in the control group(P<0.05). The height of the anterior edge of the injured vertebrae and the height of the midline of the injured vertebrae in the two groups were higher than those before treatment(P<0.05), while the recovery degree of the injured vertebral height in the experimental group was not significantly different from that in the control group(P>0.05).ConclusionFuyuan Huoxue Decoction combined with PKP is effective in the treatment of thoracolumbar compression fractures, which can significantly reduce pain,help promote the recovery of lumbar vertebtral function and alleviate clinial symptoms.

      摘要:ObjectiveTo compare the clinical efficacy of acupuncture, Chinese medicine hot compress, acunpuncture combined with moxibustion, acupuncture combined with Chinese medicine hot compress in the treatment of patients with knee osteoarthritis (KOA) of cold-damp obstruction syndrome.MethodsA total of 236 patients with KOA recruited from the Beijing Zhongguancun Hospital, Longzeyuan Community Health Service Center, Changping District of Beijing,Beixiaguan Community Health Service Center, Haidian District of Beijing from December 2020 to October 2022 were divided into acupuncture group, Chinese medicine hot compress group, acupuncture combined with moxibustion group, and acupuncture combined with Chinese medicine hot compress group according to randomly grouping, control and non-blind method in the proportion of 1∶1∶1∶1 with 59 cases in each group. The intervention of each group was delivered once daily,5 times a week, and one course of treatments was 10 times, 2 courses were required. The scores of WOMAC and VAS were compared among the 4 groups before treatment, after the first and second courses of treatment, and after follow-up for 1, 2, and 3 months, and the clinical efficacy and safety were evaluated.ResultsWOMAC score and VAS score of the 4 groups at each time point after treatment were decreased compared with those before treatment(P<0.05). After the first course of treatment,WOMAC score of acupuncture combined with Chinese medicine hot compress group was lower than that of the acupuncture group (P<0.05); after the second course of treatment and the third follow-up visit,WOMAC score of the acupuncture combined with Chinese medicine hot compress group was lower than that of the acupuncture group and Chinese medicine hot compress group(P<0.05). After the first course of treatment,VAS score of both the acupuncture combined with moxibustion group and the acupuncture combined with Chinese medicine hot compress group were lower than that of the acupuncture group(P<0.05); after the second course of treatment, VAS score of the acupuncture combined with Chinese medicine hot compress group was lower than that of the acupuncture group and Chinese medicine hot compress group(P<0.05); Three follow-up visits showed VAS score of the acupuncture combined with moxibustion group and acupuncture combined with Chinese medicine hot compress group were lower than that of the acupuncture group and Chinese medicine hot compress group (P<0.05). The clinical effect of the acupuncture combined with Chinese medicine hot compress group was superior to the other three groups(P<0.05). In the Chinese medicine hot compress group, 1 case had mild allergic reaction and was healed without any treatment.ConclusionAcupuncture, Chinese medicine hot compress, acupuncture combined with moxibustion, acupuncture combined with Chinese medicine hot compress can all achieve clinical effects in the treatment of knee osteoarthritis of cold-damp obstruction syndrome. The combination of acupuncture, moxibustion, and Chinese medicine hot compress is superior to the single therapy, and the combination of acupuncture but Chinese medicine hot compress group has more advantages.

      摘要:ObjectiveTo observe the regulatory effect of Guben Zhike Recipe on airway inflammation in mice with chronic obstructive pulmonary disease (COPD).MethodsTwenty mice were divided into a blank control group, a model group, a Guben Zhike Recipe group, and a dexamethasone group (n=5). Except for the normal group, all other mice were used to established COPD animal models by smoking combined with nasal instillation of lipopolysaccharide (LPS). After successful modeling, the traditional Chinese medicine group was given 0.2 mL/d of Guben Zhike Recipe by gavage of 0.9% physiological saline, while the blank control group and model group were given 0 9% physiological saline was administered by gavage, and the dexamethasone group was given dexamethasone injection (1.0mg/kg) intraperitoneal injection for 28 consecutive days, twice a day. During the modeling period, the general situations of the mice were recorded, and the NO content in lung tissue homogenate was detected using enzyme-linked immunosorbent assay. The expression of p38 MAPK, p-p398MAPK, and ATF-2 proteins was measured using Western blot.ResultsCompared with the model group, the NO level in the lung tissue homogenate of Guben Zhike Recipe group (for strengthening the foundation and relieving cough)was decreased(P<0.05), the expression of p38MAPK proteins decreased (P<0.01).ConclusionGuben Zhike Recipe can improve airway inflammation in stable COPD mice, and the mechanism may be related to the inhibition of the p38 MAPK pathway.

      摘要:ObjectiveTo explore the effect and mechanism of Jianpi Yiqi Shexue Prescription (JPYQSX) for immune thrombocytopenia (ITP) mice.MethodsA total of 90 SPF BALB/c mice were divided into normal control group, model group, prednisone group, Jianpi Yiqi Shexue Prescription (JPYQSX) medium and high dose group according to random number table method with 18 in each group. The normal control group was intraperitoneally injected with 100 μL/20 g of physiological saline, and the other groups were intraperitoneally injected with 100 μ l/20 g of APS once a day to establish ITP mouse models. From the 8th day of modeling, the normal control group and model group were given 0.1 mL/10 g saline by gavage, while the prednisone group and the middle and high dose groups of invigorating spleen and benefiting qi were given 0.1 mL/10 g corresponding drugs by gavage once a day for 8 days. The protein expressions of SDHA, ClpP and LonP1 in tissues and organs of mice were detected by Western blotting; the mRNA expressions of SDHA, ClpP and LonP1 detected by real-time quantitative PCR (qPCR).ResultsThe expression of SDHA protein in the spleen, heart and brain tissue of the model group was higher than that of the control group(P<0.05). The expression of SDHA protein in the spleen of the prednisone group and the JPYQSX group was significantly lower than that of the model group(P<0.05); The expression of ClpP protein in the spleen, heart and skeletal muscle of the model group was significantly higher than that of the control group(P<0.05), and the expression of ClpP in the spleen of each treatment group was significantly lower than that of the model group(P<0.05); the expression of LonP1 in the spleen tissue of the model group was significantly lower than that of the control group(P<0.05), the expression of LonP1 in the spleen tissue of the prednisone group was significantly higher than that of the model group(P<0.05), and the expression of LonP1 in the heart and skeletal muscle of the model group was significantly higher than that of the control group(P<0.05), and the LonP1 expression in skeletal muscle of the prednisone group was significantly lower than that of the model group(P<0.05). The protein detection results were basically consistent with the transcription level detection results of the corresponding tissues.ConclusionJPYQSX can regulate mitochondrial function in ITP mice so as to relieve fatigue.

      摘要:ObjectiveTo explore the molecular mechanism of Zizao Yangrong Decoction (ZYD) in treating skin xerosis caused by anti-cancer targeted drugs based on network pharmacological analysis and animal experiments.MethodsOnline databases were used to search for the corresponding targets of each single herb included in ZYD and skin xerosis and pruritus. The shared targets were analyzed and used to predict the possible effect pathways through pathway enrichment. Rat model consistent with patients' dry skin lesions by targeting drug gefitinib was established, and were divided into model group, positive control group and Chinese herb group, which were treated by normal saline, vitamin E cream, ZYD respectively. Compared with normal rat, the skin phenotype, histopathological changes, the expression of signal pathway-related proteins and inflammation factors were observed.ResultsSingle herb of ZYD was screened out a total of 3125 predicted targets. Targeted drugs-induced skin xerosis which was accompanied by pruritus and caused by targeted drugs was screened out a total of 660 targets. 173 targets shared by ZYD and diseases were analyzed. Pathway enrichment showed that PI3K-Akt pathway might be the signal pathway of the effect of ZYD. Animal experiments showed that compared with normal group, rats in other groups developed target drug-related skin xerosis like lesion, scrab and desquamation. Compared with model group, the dryness of skin condition of rats in positive control group and experimental group were improved. The improvement of skin healing and hair growth in experimental group were better than that in positive control group. In the model group, Akt protein and p-Akt 308 protein were stained brown and the intensity was weak. Compared with the model group, Akt protein and p-Akt 308 protein in the Chinese herb group were brown, and the intensity was stronger than that in the positive control group, which was similar to that in the normal control group. There was no significant difference in the staining intensity of p-Akt 473 protein among the groups. Western blotting results: compared with the normal control group, the relative expression of p-Akt 308 protein in the skin tissue of Chinese medicine group was lower(P<0.05); compared with the model group, the relative expression of p-Akt 308 protein in the skin tissue of Chinese herb group was higher(P<0.05). There was no significant difference in the levels of inflammatory factors in skin tissue and serum among the groups(P>0.05).ConclusionChinese herb ZYD might have a therapeutic effect on targeted drugs-associated skin xerosis by relieving inflammatory reaction and promoting skin recovery, in which Akt-related pathway might be involved.

      摘要:ObjectiveTo explore the effect and mechanism of panax notoginseng(PN)on methicillin-resistant staphylococcus aureus(MRSA) cell wall.MethodsThere were PN treatment group and control group in this study. MRSA in control group was not intervened, while MRSA in PN group was intervened by 2 mg/mLPN for 24 hours. The effects of PN on wall peptidoglycan(PGN)and wall phosphate(LTA)in MRSA cells were detected using ELISA. The effect of PN on the thickness of MRSA cell wall was observed through transmission electron microscopy. Genes related to the efficacy of PN were screened using RNA Seq technology,then these genes were verified by RT-PCR.ResultsCompared with the control group,the levels of PGN and LTA in MRSA in PN group were significantly reduced (P<0.05). In PN group, the number and volume of bacteria in dividing stage increased, the dividing membrane of bacteria was blurred and the cell wall thickness was thin (P<0.05). There were 552 different genes between the two groups. Compared with the control group, 285 genes in PN group were up-regulated and 267 genes were down-regulated. Biological processes (BP) related to differential genes are mainly concentrated in redox, precursor metabolites and energy production, cellular amino acid catabolism and aerobic respiration, cellular components (CC) are mainly concentrated in cellular anatomical entities, cellular organs and non-membrane organelles, and molecular functions (MF) are mainly concentrated in redox enzyme activity, structural molecular activity and electron transfer activity. The differential genes enriched 85 signal pathways, including fatty acid and lysine degradation. The differential genes MraY, MurD, MurC, FemA, FemB and sgtB are related to MRSA cell wall. Compared with the control group, the expressions of MraY, MurD, FemA and FemB in PN group decreased (P<0.05), while the expressions of MurC and sgtB increased(P<0.05).ConclusionPN exerts the therapeutic effect on MRSA by inhibiting its cell wall which is related to reducing the expression of cell wall-related genes and promoting the degradation of fatty acids and lysine.

      摘要:On the basis of clinical observation combined with analogism, helicobacter pylori can be classified into an insect pathogen with damp heat properties, namely damp heat insect pathogen. The key to the onset of helicobacter pylori infection is the invasion of damp heat insect pathogen into the stomach, and the weakness of spleen and stomach or damp heat of spleen and stomach provide internal conditions for the invasion of damp heat insect pathogen. After the colonization of damp heat insect pathogen, it will affect the circulation of qi and blood in the stomach and produce insect toxins that damage the human body. In the initial stage, the pathogen is mild and shallow, the body can still tolerate it. Over time, the disease pathogens get deepened. The pathogenic process of dampness and heat, from the shallow to the deep and from the influence of intangible qi to the endogenous generation of tangible evil, can be seen in the stagnation of insect evil, the disharmony of stomach qi, the stagnation of evil in the stomach, the brewing of heat, the disharmony of qi and blood, and the combination of insect and toxin, as well as the internal injury of the spleen and stomach and the depletion of qi and yin. The treatment principles of descending stomach qi, clearing heat and dampness, promoting blood circulation and detoxification, and tonifying the spleen and stomach are adopted respectively.

      摘要:Viral myocarditis (VMC) is mostly due to infection with cardiomyopathic virus, which causes immune-inflammatory reactions leading to myocardial lesions, and atrial fibrillation is one of its main clinical manifestations. The syndrome differentiation and treatment of traditional Chinese medicine has certain advantages in the treatment of viral myocarditis, and the main pathogenesis of viral myocarditis atrial fibrillation is deficiency of anti-pathogenic qi and attack of external wind, and it is also related to dampness, heat, stasis, toxicity, and deficiency, and viral myocarditis atrial fibrillation can be identified and treated by four stages, namely, acute, remission, recovery, and chronic phases.
