      摘要:ObjectiveTo observe the therapeutic effect of Changyanning Tablets on colorectal adenoma-carcinoma (CRA-CRC) model rats and explore its possible mechanisms.MethodsSix rats were randomly assigned to the normal group (Normal). The remaining rats were intraperitoneally injected with 1,2-dimethylhydrazine (DMH) at 60 mg/kg once a week for 15 weeks to establish the CRA-CRC model. In the 7th week, the model rats were randomly divided into five groups according to a randomized block design: a model group (Model), a berberine group (HLS), a low dose group of Changyanning Tablets (L-CYN), a medium dose group of Changyanning Tablets (M-CYN), and a high dose group of Changyanning Tablets (H-CYN), with 6 rats in each group. From the 7th week of modeling, the HLS group was given berberine at 0.027 g/kg by gavage, while the L-CYN, M-CYN, and H-CYN groups were given Changyanning Tablets at 0.3, 0.6, and 1.2 g/kg by gavage, respectively. The Normal and Model groups were given distilled water by gavage. The above drug treatments were performed once daily for 8 weeks. General conditions, body weight, abdominal anatomical structure, incidence of intestinal tumors, pathological changes in intestinal tumor tissues, vascular endothelial growth factor (VEGF) protein expression in colorectal tissues, and cleaved Caspase-3 expression were compared among the groups.ResultsBy the 15th week of modeling, compared with the Normal group, the Model group had a lower body weight (P<0.05). No significant difference in body weight was observed in the HLS, L-CYN, M-CYN, and H-CYN groups compared with the Model group (P>0.05). The Model group showed a large amount of ascites. No obvious organ abnormalities or tumor metastasis were observed in the abdominal cavity of rats in any group. Compared with the Normal group, the Model group had a shorter colorectal length (P<0.05) and a higher number of tumors (P<0.01). Compared with the results in the Model group, the colorectal length in the H-CYN group was longer (P<0.05), and the number of tumors was lower in the HLS, H-CYN, and M-CYN groups (P<0.05), with one rat in the H-CYN group showing no visible tumors. Tumor size was larger in the Model group than in the Normal group (P<0.01), and smaller in the HLS, H-CYN, and M-CYN groups (P<0.01). The Model group showed significant increases in gland and cell numbers in the colon, extensive inflammatory cell infiltration between cells, dense cell arrangement, and significant changes in tumor cell size and morphology, including large and deeply stained nuclei, frequent nuclear division, and focal glandular severe atypical hyperplasia, high-grade intraepithelial neoplasia, and focal in situ carcinoma formation. In the L-CYN group, goblet cells disappeared, cells proliferated and were closely arranged, with obvious nuclear division and numerous small nucleoli. The HLS and M-CYN groups had similar pathological results, including lymph follicle hyperplasia, glandular repair-like changes, crypt abscesses, and focal reduction of inherent glands. The H-CYN group had more organized gland arrangement, focal inflammatory cell infiltration, erosion, and focal reduction of inherent glands, with occasional nuclear division. The Model group had an incidence of adenocarcinoma of 50% and tubular adenoma of 33.3% (with mild and moderate atypical hyperplasia), and one rat with severe atypical hyperplasia. All rats were successfully modeled. The pathological changes in intestinal tumor tissues in the L-CYN group were not significantly improved, whereas the HLS, M-CYN, and H-CYN groups showed reduced tumor tissue lesions, with the H-CYN group showing the most significant effect. Compared with the results in the Model group, VEGF protein expression was lower in the L-CYN group, though not statistically significant (P>0.05), and significantly lower in the HLS, M-CYN, and H-CYN groups (P<0.05). Cleaved Caspase-3 protein expression was lower in the colorectal tissues of the Model group than that in the Normal group (P<0.05). Cleaved Caspase-3 protein expression was higher in the colorectal tissues of the Chinese medicine treatment groups and the HLS group than that in the Model group (P<0.05,P<0.01).ConclusionChangyanning Tablets can improve clinical symptoms and pathological changes in the colorectal mucosa of CRA-CRC model rats, and its mechanism may be related to the negative regulation of VEGF expression and the promotion of cleaved Caspase-3 expression.

      摘要:ObjectiveTo explore the intervention effect of Jianpi Qingre Huoxue Formula (JQH) on gastric cancer cells.MethodsThe intervention effect of JQH on gastric cancer cells was investigated using CCK-8 assays, apoptosis flow cytometry, scratch assays, and nude mouse subcutaneous tumor formation tests. Network pharmacology was employed to identify its potential mechanisms of action.ResultsAfter CCK-8 detection, the proliferation rate of JQH intervention group was lower than blank group at 24, 48 and 72 h. Flow cytometry showed that the percentage of apoptotic cells in JQH intervention group was higher than blank group. By scratch test, the healing percentage (migration ability) of gastric cancer cells in JQH intervention group at 24 and 48 h was lower than blank group. The volume and weight of subcutaneous tumor of nude mice in the JQH intervention group were lower than those in the blank group, the antitumor effect of medium and high dose groups was similar to that of 5-Fu group. Network pharmacology indicated that JQH could intervene in gastric cancer cells through multiple targets. Its mechanisms of action may involve regulation of phosphorylation, protein phosphorylation, cell proliferation, and apoptosis, and it may inhibit cancer by modulating the PI3K/AKT pathway.ConclusionJQH can affect the proliferation, migration and apoptosis of gastric cancer cells through multiple pathways and pathways.

      摘要:Ulcerative colitis (UC) is a recurrent inflammatory bowel disease. Current research suggests that its pathogenesis is closely related to factors such as intestinal epithelial barrier defects, gut microbiota dysbiosis, immune response disorders, and genetics. Baitouweng Decoction has shown certain efficacy in treating UC. Baitouweng Decoction and its modified formulas can reduce the expression of pro-inflammatory cytokines through multiple signaling pathways, increase anti-inflammatory factors, promote the proliferation of intestinal epithelial cells to accelerate epithelial repair, protect the intestinal mucosal barrier, and alleviate intestinal inflammation. In this way, they reduce abnormal proliferation of intestinal epithelial cells and prevent the occurrence of inflammation-induced cancer.

      摘要:Professor WEI Wei believes that “deficiency, stagnation, depression, and stasis” are the core pathogenesis of the occurrence and development of precancerous lesions of chronic atrophic gastritis. The onset of this disease is mainly related to factors such as spleen and stomach deficiency, obstruction, emotional depression, and collateral stasis. Based on syndrome differentiation and treatment, modern scientific and technological methods are widely absorbed and adopted. By integrating the core pathogenesis of “deficiency, stagnation, depression, and stasis” with its material basis, gastroscopy results, and pathological findings, a systematic exploration of the relationship between the material basis of precancerous lesions of chronic atrophic gastritis and the microscopic syndrome differentiation of traditional Chinese medicine (TCM) is conducted from multiple perspectives, combining both macro and micro approaches. It enables the realization of an objective and standardized diagnosis and treatment strategy in TCM, providing a reference for the theoretical interpretation of the pathogenesis and precise clinical syndrome differentiation and treatment of this disease.

      摘要:ObjectiveTo evaluate the efficacy of modified Baihe Dihuang Decoction and Ganmai Dazao Decoction in the treatment of insomnia in patients with atrial fibrillation of yin deficiency and fire flourishing type.MethodsThe patients with atrial fibrillation of yin deficiency and fire flourishing type, who visited the Outpatient Department,South Campus of Guang 'anmen Hospital of China Academy of Chinese Medical Science from January 2019 to December 2020, were selected. According to the simple random method, the patients were devided into observation group and control group.Each group was treated with regular western medicine to control atrial fibrillation.The treatment group was treated with modified Baihe Dihuang Decoction and Ganmai Dazao Decoction in plus.The control group was treated with Oryzanol with 20 mg/time,3 times/d.The treatment courses of the two groups were four weeks. Pittsburgh Sleep Quality Index (PSQI) score, Sleep Status Self-Rating Scale (SRSS) score and curative effect were compared between the two groups before and after treatment.ResultsAfter treatment, the PSQI scores of both groups decreased compared to before treatment (P<0.05), and the observation group was lower than the control group (P<0.05); The SRSS scores of both groups decreased compared to before treatment (P<0.05), and the observation group was lower than the control group (P<0.05). There was a statistically significant difference in the efficacy of PSQI and SRSS between the two groups (P<0.05). The observation group showed better efficacy in treating multiple dreams, easy awakening compared to the control group (P<0.05).ConclusionThe combination of modified Baihedihuang Decoction and Ganmaidazao Decoction can significantly improve insomnia symptoms in patients with yin deficiency and fire hyperactivity type atrial fibrillation, and its therapeutic effect is superior to that of oryzanol.

      摘要:ObjectiveTo observe the effect of preoperative electroacupuncture (EA) pretreatment on postoperative analgesia in patients undergoing total knee arthroplasty (TKA) under general anesthesia.Methods96 patients undergoing their first unilateral TKA under general anesthesia at Xuzhou Central Hospital were enrolled and randomly divided into an observation group and a control group according to a random table number, with 48 cases in each group. The observation group received EA pretreatment combined with local infiltration anesthesia and patient-controlled intravenous analgesia (PCIA), while the control group received local infiltration anesthesia and PCIA. Plasma levels of bradykinin (BK), prostaglandin E2 (PGE2), substance P (SP), β-endorphin (β-ep), and dynorphin (Dyn) before and after surgery, and VAS scores at 12, 24, and 48 hours postoperatively, as well as anesthesia drug usage and adverse reactions, were compared between the two groups.ResultsPostoperatively, plasma levels of BK, PGE2, SP, β-ep, and Dyn in both groups differed significantly from their preoperative levels(P<0.01). The observation group had lower plasma levels of BK, PGE2, and SP than the control group (P<0.05), and higher levels of β-ep and Dyn (P<0.05). For resting VAS scores at 12, 24, and 48 hours postoperatively, there was no significant difference between the two groups (F=0.694,P=0.406), but intra-group comparisons at each time point showed significant differences (F=256.6,P<0.01). For activity VAS scores at 12, 24, and 48 hours, significant differences were observed between the groups (F=7.072,P=0.008), and intra-group comparisons at each time point also showed significant differences (F=300.885,P<0.01). The observation group had a later first press time than the control group (P<0.01), fewer total PCIA presses(P<0.01), and fewer cases requiring additional analgesia, though this difference was not statistically significant(P>0.05). The incidence of nausea and vomiting was lower in the observation group than the control group(P<0.05), but there were no significant differences in the incidence of dizziness, drowsiness, respiratory depression, or itching between the two groups(P>0.05).ConclusionEA pretreatment enhances the analgesic effect of local infiltration anesthesia combined with PCIA in patients undergoing TKA.

      摘要:ObjectiveTo observe the clinical efficacy of a self-made Chinese medicine gargle in the treatment of periodontitis with intense stomach fire syndrome.MethodsA total of 100 patients with chronic periodontitis of intense stomach fire syndrome, treated at the Department of Stomatology, Guang'anmen Hospital South District of the China Academy of Chinese Medical Sciences from March 2023 to March 2024 were enrolled and randomly divided into two groups using a random number method. Both groups received oral hygiene education and basic periodontal treatment. The observation group gargled with a self-made Chinese medicine solution for 2 min, 25–30 mL per use, 3 times a day. The control group gargled with 0.9% saline solution following the same method as the observation group. Both groups continued gargling for 2 weeks. The levels of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), high-sensitivity C-reactive protein (hs-CRP), gingival index (GI), sulcus bleeding index (SBI), and traditional Chinese medicine (TCM) syndrome scores were compared between the two groups before and after treatment.ResultsAfter treatment, the levels of CRP, IL-1β, TNF-α, GI, SBI in gingival crevicular fluid, and the scores for gingival swelling, bleeding, halitosis, and constipation in both groups were lower than those before treatment (P<0.05), and the observation group showed significantly lower levels than the control group (P<0.05).ConclusionThe self-made Chinese medicine gargle can reduce the periodontal inflammatory response in patients with periodontitis of intense stomach fire syndrome and improve periodontal status and clinical symptoms.

      摘要:ObjectiveTo observe the effect of Jianpi Zhidong Decoction on intrastriatal dopamine (DA) vesicle circulation of Tourette syndrome (TS) model rats.MethodsForty-two rats were randomly divided into a blank group (n=10) and an experimental group (n=32) using a complete randomization method. The experimental group was given intraperitoneal injections of iminodipropionitrile (IDPN) to establish the model, while the blank group was given intraperitoneal injections of an equal volume of saline, once daily for 7 consecutive days for both groups. The 30 successfully modeled rats were then randomly divided into a model group, a Jianpi Zhidong Decoction group (TCM group), and a Tiapride group using a complete randomization method, with 10 rats in each group. The TCM group was given Jianpi Zhidong Decoction granules at 1.6 g/100 g by gavage, and the Tiapride group was given Tiapride suspension at 2.1 mg/100 g by gavage. The blank and model groups were both given saline at 1 mL/100 g by gavage once daily for 4 consecutive weeks. Stereotyped behavior scores in all groups were compared on days 1,8,15,22,29, and 36. The expression of tyrosine hydroxylase (TH), dopamine transporter (DAT), vesicular monoamine transporter 2 (VMAT2), and α-synuclein (α-syn) in the striatum was also measured and compared.ResultsBy day 8, at the end of modeling, the stereotyped behavior scores in the model group and the Tiapride group were higher than those in the blank group (P<0.05). From day 22 onward, the stereotyped behavior scores in the TCM and the Tiapride groups were lower than those in the model group (P<0.05), with no statistically significant difference between the TCM and the Tiapride groups at any time point (P>0.05). Compared with the stereotyped behavior scores on day 1, those in the model, TCM, and Tiapride groups increased on days 8 and 15 (P<0.05). However, compared with the stereotyped behavior scores on day 8, those in the TCM and Tiapride groups decreased from day 29 onward (P<0.05). Compared with the blank group, the model group had higher TH protein expression and lower DAT protein expression in the striatum (P<0.05). Compared with the model group, both the TCM and the Tiapride groups had lower TH protein expression (P<0.05), while the TCM group had higher DAT protein expression (P<0.05). Additionally, compared with the blank group, the model group had higher α-syn protein expression and lower VMAT2 protein expression in the striatum (P<0.05). Compared with the model group, both the TCM and the Tiapride groups had lower α-syn protein expression and higher VMAT2 protein expression (P<0.05).ConclusionJianpi Zhidong Decoction exerts an anti-tic effect by regulating DA vesicle circulation in the striatum of TS model rats, inhibiting excessive α-syn release, increasing the expression of DAT and VMAT2, and reducing DA content in the synaptic cleft of the brain.

      摘要:Fire needle therapy has a wide range of indications and complex mechanisms. The research on its treatment of common diseases mainly focuses on skin, musculoskeletal system, and neurological disorders, reflecting multi-level, multi-target, multi-pathway, and multi-link characteristics. The primary mechanisms include ①anti-inflammatory and immunoregulatory effects;②regulation of cell proliferation, differentiation, and apoptosis;③endocrine modulation;④improvement of blood circulation and vascular repair;⑤regulation of neurotransmitters and neuropeptides;⑥inhibition of oxidative stress;⑦repair of skin lesions;⑧nerve protection. Future research should aim to improve methodological and reporting quality in clinical mechanism studies, develop standardized fire needle instruments, and develop standard treatment protocols for different diseases. It is crucial to explore the thermal conduction mechanism of fire needle therapy, compare its therapeutic effects under various conditions, objectively evaluate its safety, uncover its peripheral and central mechanisms by virtue of diverse medical imaging technologies, and provide a comprehensive and dynamic explanation of the overall effects by omics technologies from systems biology.
