Elderly patients are the main group of critically ill patients with Corona Virus Disease 2019 for they have the physical characteristics of five zang organs deficiency,attack between the true and the evil,and emotional fluctuation.In this epidemic,"dampness","poison" and "deficiency" are mixed with each other.TCM differentiation of syndrome and treatment should focus on the theory of Wei Qi and Ying blood,Sanjiao theory,and pivot function of Shaoyang should be also taken into account.Based on the clinical experience of hospitalized elderly patients,this problem is enplaned by dividing into four phases and seven syndromes.At the initial stage,many toxic substances are trapped in the membrane and the middle energizer,or wet toxic substances invade into the Wei Qi and overflow to the channels and collaterals,or toxic substances are trapped in the Shaoyang.In the advanced stage,transformation of dampness and toxin into heat which fumigate and burn qi,or transformation of dampness and toxin into heat entering the Ying and injuring blood are commonly seen.During the critical period,the pathogenic factors are excessive and the anti-pathogenic qi becomes weak,or even internal closure with external detachment appears.During the recovery period,the deficiency of both qi and yin can be seen,and the damp toxin remains.The treatment should "attaching importance to pathogenic damp","penetrating detoxification" and "giving attention to body deficiency".Elderly patients with Corona Virus Disease 2019 and normal patients have common in pathogenesis and nature,but obvious individual differences are founded.Clinically,we should follow the method of special prescriptions for specific diseases as well as individualized syndrome differentiation and treatment.
