摘要

“二阳结谓之消，三阳结谓之膈”，出自《素问·阴阳别论篇》，与糖尿病肾病（diabetic kidney disease， DKD）的病机演变相契合，对DKD的治疗起到指导作用。在饮食、情志、禀赋影响下，阳结胃肠即成“二阳结”，阳邪销烁津液、耗气伤津而成消渴，是DKD的始动因素；治疗当益气清热兼以润燥、祛风。“二阳结”失治误治，阳邪久羁胃肠，可化生痰、湿、瘀阻碍小肠、膀胱气化，而成“三阳结”，气化不利进一步加重痰、湿、瘀等病理改变，终致“肾络癥瘕”形成；治疗当益气消癥，配合化痰、除湿、活血、通络。

Abstract

Traditional Chinese medicine （TCM） has a long history in the diagnosis and treatment of diabetic kidney disease （DKD）. The phrase "When pathogenic factors are stagnated in two

ficulty in defecation and urinationis caused" originates from Plain Questions：Separate Discussion on

） and corresponds closely with the pathogenesis and progression of DKD， providing guidance for its treatment. Under the influence of diet， emotions， and constitution，

yang

congeals in the stomach and intestines， forming the "two

yang

congealment" stage.

Yang

evil consumes body fluids and injures

， resulting in thirst disease， which is the initiating factor of DKD. Treatment should focus on tonifying

， clearing heat， nourishing dryness， and dispelling wind. If the "two

yang

congealment" stage is untreated or improperly treated，

yang

evil lingers in the stomach and intestines， generating phlegm， dampness， blood stasis， and deficiency that obstruct the small intestine and bladder

transformation， progressing to the "three

yang

congealment" stage. Impaired qi transformation further aggravates phlegm， dampness， stasis， and deficiency， eventually leading to the formation of "kidney collateral abdominal mass" syndrome. Treatment should focus on tonifying

and resolving mass， combined with eliminating phlegm， removing dampness， activating blood circulation， and dredging collaterals.
